About this resource
Everything inside: 21 pages, 507 type-in fields · four genuinely different ways to collect self-help data (a weekly frequency tally by routine area, a 14-opportunity structured trial log, a quick daily snapshot across four routine areas, and a plain-words paraprofessional/general-education form) · a 4-routine-area, 4-support-level, 4-setting code key · a whole-caseload roster · a binder cover and 4 cut-out tabs · a cumulative tracker and graph · a quarterly summary · a family update · a 20-starter phrase bank · a worked, invented example.
The store's only PreK-specific data file folds self-help into the top half of one shared 8-row table alongside three other developmental areas. This file is the self-help-only, deeper companion: four routine areas a team watches at this stage — dressing, feeding/mealtime, the toileting/hygiene routine, and cleanup/transition — recorded as plain classroom/caregiver observation, never a therapy procedure. Run the form that fits the moment, mark only what was actually observed, roll the week into a tracker and a quarterly report, and send a family update that is ready to go.
WHAT IS INCLUDED — 21 pages
Set Up The Caseload
- Code Key: Routine Areas, Support Levels and Settings — 4 routine-area codes with a plain description of what each looks like, 4 support/response codes, 4 setting codes, a 3-item "before the first cycle" setup checklist, and a blank line for your team's own criteria
- PreK Self-Help Caseload Roster — 14 rows: child, group/classroom, routine area(s) tracked, typical support level, setting, next review due
- Binder Cover and Divider Tabs — a spine/cover card, 4 cut-out tabs matching the file's sections, and a 10-row "where to find things" index
TAKE THE DATA (four genuinely different forms)
- Daily / Weekly Self-Help Frequency Tally — one row per routine area (4 rows) across 5 days plus a week-total row, with a worked "3 (I)" format example, and an optional 4-week quick-trend table (4 rows) for a fast read before a meeting
- Structured Self-Help Opportunity Trial Log — a 14-opportunity working bank (4 dressing, 4 feeding/mealtime, 3 toileting/hygiene, 3 cleanup/transition) with the opportunity and the target response printed ahead of time, and a row for what the child actually did and the support-level mark
- Quick Daily Self-Help Snapshot — a fast, four-routine-area, Y/N/NOT OBSERVED weekly grid (a simpler code set than the trial log, defined on the page), plus a "move to the detailed trial log when" checklist
- Paraprofessional and General Education Self-Help Form — 12 rows, plain words instead of codes, for the adult in the room who is not the case manager
Track, Report And Share
- Cumulative Self-Help Tracker — two 12-session blocks (routine area 1, and an optional routine area 2), with date, percent independent and setting logged each session
- Progress Graph — a scaled 0–100 grid to plot the trend, with an aim line
- Quarterly Progress Summary — 6 rows (baseline, current level, one sentence per routine area), plus settings, percent then/now, and two write-on cards
- Family Update — Self-Help and Adaptive Skills — a ready-to-send communication page: what was worked on and the routine area, how the child did it most this week, something that went well, one thing to try at home, family questions and answers, and a 7-day practiced table
- Progress Note Phrase Bank — 20 observable-language starters across 5 groups (dressing/feeding, toileting/hygiene, cleanup/transition, next steps, the quarterly-report sentence), every one ending in a blank pulled from a data sheet in the file
Plus a Start Here quick-start, a 3-section contents page with real page thumbnails and a routine-area-code legend, 3 section dividers, and a "what's next" back page naming the other three PreK products in this collection.
What This Is, And What It Is Not
It is a fillable PDF. Open it in a PDF reader, click a field, type, Save As once per child. It works offline with no account or subscription. It is NOT a Google Sheets file, a Google Doc, or an Excel workbook, and it does not compute a percent or decide what counts as a routine being completed for you. It is NOT an occupational or physical therapy tool and does not describe a physical-prompting technique: a "P" mark only shows a prompt was given, and the team writes what kind in its own words.
It is not the top half of the single shared self-care log inside Preschool and Early Childhood Progress Monitoring Data Sheets Editable — that is one 8-row table among four developmental areas. This file is the self-help-only, deeper companion. It is also not Functional Living Skills and Adaptive Behavior Data Sheets Editable, the school-age, ongoing sibling built around six broader independent-living domains (self-care, household, community, money/safety, leisure), and not Life Skills and Daily Living Data Sheets Editable, which is curriculum/task-analysis planning content rather than a short, repeatable data file. No page in this file states an age a routine "should" appear by, names a condition, or sets a rule for when a referral is warranted; every criterion is a blank line the team writes in from the IEP or plan and its own judgment. The opportunities, codes and routine areas on these pages are built for this file, not a standardized instrument. No health data is pre-filled anywhere, and the example child (Mateo) is invented and named as such on page 20.
Who This Is For
Early-childhood special educators, interventionists, and PreK case managers with a self-help or adaptive-skills goal on the caseload, plus the paraprofessionals and general education teachers who help collect the data.
If You Want More
Preschool and Early Childhood Progress Monitoring Data Sheets Editable is the short, four-domain overview for a whole PreK caseload. PreK Communication Skills, PreK Pre-Academic Skills, and PreK Social-Emotional and Play-Based Learning Progress Monitoring Data Sheets Editable are the other three deeper, single-domain companions in this same collection.
