About this resource
Everything inside: 21 pages, 552 type-in fields · three genuinely different ways to collect social-play data (a weekly frequency tally by skill and setting, a 12-opportunity structured peer-interaction log, and a quick daily snapshot across six skills), plus a plain-words paraprofessional/general-education form · a 6-skill, 4-setting code key · a whole-caseload roster · a binder cover and 4 cut-out tabs · a cumulative tracker and graph · a quarterly summary · a family update · a 20-starter phrase bank · a worked, invented example.
WHAT IS INCLUDED — 21 pages
Set Up The Caseload
- Code Key: Settings, Support Levels and Skills — 4 setting codes, 4 support/response codes, 6 social-play skill codes with a plain description of what each looks like, a 3-item "before the first cycle" setup checklist, and a blank line for your team's own criteria
- PreK Social-Emotional and Play Caseload Roster — 14 rows: child, group/classroom, skill(s) tracked, primary setting, support level usually needed, next review due
- Binder Cover and Divider Tabs — a spine/cover card, 4 cut-out tabs matching the file's sections, and a 10-row "where to find things" index
TAKE THE DATA (three genuinely different forms, plus a second-audience form)
- Daily / Weekly Social-Play Frequency Tally — one row per skill (6 rows) across 5 days plus a week-total row, with a worked "3 (FP)" format example, and an optional 4-week quick-trend table (6 rows) for a fast read before a meeting
- Structured Peer-Interaction Opportunity Log — a 12-opportunity working bank (2 opportunities for each of the 6 skills) with the opportunity and the target response printed ahead of time, and a row for what the child actually did and the support-level mark
- Quick Daily Social-Play Snapshot — a fast, six-skill, Y/N/NOT OBSERVED weekly grid (a simpler code set than the opportunity log, defined on the page), plus a "move to the detailed opportunity log when" checklist
- Paraprofessional and General Education Social-Play Form — 12 rows, plain words instead of codes, for the adult in the room who is not the case manager
Track, Report And Share
- Cumulative Social-Play Tracker — two 12-session blocks (skill 1, and an optional skill 2), with date, percent independent and setting logged each session
- Progress Graph — a scaled 0–100 grid to plot the trend, with an aim line
- Quarterly Progress Summary — 6 rows (baseline, current level, one sentence per skill), plus settings, percent then/now, and two write-on cards
- Family Update — Social-Emotional and Play Skills — a ready-to-send communication page: what was worked on and the skill, how the child showed it most, something that went well, one thing to try at home, family questions and answers, and a 7-day practiced table
- Progress Note Phrase Bank — 20 observable-language starters across 5 groups (joining play/responding to peers, turn-taking/sharing, routines/calming, next steps, the quarterly-report sentence), every one ending in a blank pulled from a data sheet in the file
Plus a Start Here quick-start, a 3-section contents page with real page thumbnails and a skill/setting-code legend, 3 section dividers, and a "what's next" back page.
What This Is, And What It Is Not
It is a fillable PDF. Open it in a PDF reader, click a field, type, Save As once per child. It works offline with no account or subscription. It is NOT a Google Sheets file, a Google Doc, or an Excel workbook, and it does not compute a percent or decide what counts as a skill being met for you.
It is not the single shared "Self-Care and Social-Emotional / Play Checklist" bottom half inside Preschool and Early Childhood Progress Monitoring Data Sheets Editable — that is 8 rows on half a page among four developmental areas. This file is the social-emotional/play-only, deeper companion. It is also not Social Skills Progress Monitoring and Peer Interaction Data Sheets Editable, the general, school-age sibling built around a friendship rubric, conversation skills and conflict-resolution logging. No page in this file states an age a skill "should" appear by, names a condition, or sets a rule for when a referral is warranted. The "calming with adult support" skill records only whether the child returned to a calm state with the support that was given — never the calming technique or procedure itself, which stays entirely with the team's own plan. The opportunities, codes and skills on these pages are built for this file, not a standardized instrument. No health data is pre-filled anywhere, and the example child (Nora) is invented and named as such on page 20.
Who This Is For
Early-childhood special educators, counselors, and PreK interventionists with a social-emotional or play-based goal on the caseload, plus the paraprofessionals and general education teachers who help collect the data.
