About this resource

A fillable, plain-language data-collection file built for a PreK or Early Intervention caseload — shorter and simpler than the store's school-age IEP data sheets on purpose: a chooser, a caseload roster, three data sheets across four developmental areas, a tracker, a graph, and a family update note, all in one PDF you type into and print.

WHAT IS INCLUDED - 12 pages, 477 type-in fields

What This Is, And What It Is Not

It is a fillable PDF. Open it in a PDF reader, click a line, a box or a cell, type, press Tab to move on, and Save As once per child. It works offline with no account or subscription. It is NOT a Google Sheets file, a Google Doc or an Excel workbook, and it does not calculate a percent or draw the graph.

It is not a developmental checklist tied to any age, not a screening or eligibility tool, and not a curriculum. No page states an age at which a skill should appear. The skills being watched, the prompt levels and the setting codes are written in by the team, from the IEP or the intervention plan; the sheets only make sure whoever watches writes it down the same way every time.

Who This Is For

Early-childhood special educators, SLPs, interventionists and paraprofessionals working with a PreK or Early Intervention caseload who need one short, simple file for communication, motor, self-care and social-emotional or play-based progress data — not the twenty-plus-page school-age data files built for a K-12 caseload.
