About this resource
The present levels box on the IEP is written for reading, writing and math, and then the SLP, the OT, the PT and the counselor are each asked for a paragraph with nothing built for them. This is that paragraph, as a fillable PDF: an inventory of what data you actually have, seven present level templates with the sentence frames already written, a strengths bank, frames that tie a measured skill to a real class period, a worksheet that turns one row of data into one sentence, and a self-check. Type the sentences, Save As per student, paste into the district form.

How It Works
- How a Related-Service Present Level Is Built - the four parts (data, strengths, needs, effect on participation) in a four-row table that says where each one is written, plus a five-row table matching each provider to their template, the rule that every sentence carries a number and a date, and a plain statement of what this file does not do

What This Is, And What It Is Not
It is a fillable PDF. Open it in a PDF reader, click a line or a cell, type, press Tab to move on, and Save As once per student. It works offline with no account or subscription. It is NOT a Google Sheets file, a Google Doc or an Excel workbook, and it does not paste itself into your district's system.
It is not an evaluation, not a report template, and not a decision about services. Every measure is written as the evaluation or the team reported it, with the figure left blank for you to fill. No norm and no cutoff is printed anywhere, no number is called high or low, no label is named, and nothing in the file recommends a therapy, a minute count or an approach - those belong to the team and to other parts of your district's form. Your district's form decides the headings; this decides what goes under them.
