About this resource
Every adult in the room needs the same seven prompt levels, and every trial needs one mark. This editable PDF puts the levels on a poster, a blackline, reference cards and bulletin-board strips, then gives you a trial sheet, a weekly tracker and a skill summary to record and add up what happened.
EVERYTHING INSIDE (18 pages)
- Start Here: a table that lists every page, with its number and what it is for.
- How to use it: five steps, print tips, and a line for your team's own rule.
- A full-color poster of the seven prompt levels, Independent to Full physical, one plain line each, with a blank line for your team's definition.
- The same seven levels in black and white, with a line for an example from your class.
- Four reference cards for teacher prep: two level cards, a recording card and a questions card.
- Seven bulletin-board strips, colored to match the poster.
- A prompt-level trial sheet with a Name line, one check box for each level and 12 trial rows.
- A weekly prompt-fading tracker: nine skill rows by five weeks, with a line for your team's rule.
- A skill-by-skill summary with eight skill rows and a totals row.
- A staff and para quick reference with a note line for each level.
- A team hand-off and meeting notes page with four hand-off rows.
- A filled example: an invented student, an invented staff member and 10 invented trials, with the totals added up.
- A phrase bank of 8 sentence starters in 4 groups, plus 6 lines for your own starters.
- A binder cover and spine, and 8 divider tabs (teacher prep).
- What is next, terms of use, and credits.
How To Edit
Open the PDF in Adobe Acrobat Reader, Preview or your browser. Click a line or a box and type. Save As to keep a clean blank copy for next week. This is a fillable PDF, not an online tracker: nothing to log into and nothing to unlock. A check box is a small box you type an X into.
Print Tips
Print on letter paper at 100 percent. The lines and boxes are dark, so the pages photocopy in black and white. Use the blackline pages to save color ink. Print the reference cards and the binder pieces on cardstock.
The Team Writes The Rules
The file names the seven levels and gives you places to record them. It does not say which target to teach, which level to start at, or when to change a level. Those rules go on the blank lines, in your team's own words.
Student Names Stay Private
Who This Is For
Special education teachers, paraprofessionals and the teams who write and run prompt-based trials, from a single classroom to a whole program.
What This Is, And What It Is Not
A set of fillable forms and posters for recording prompt levels. It is not a program or a curriculum, and it does not tell you how to give any prompt. The example student, staff and trials are invented, and each example page says so in a footnote. It is not a gradebook and it contains no health information.
