The reading interventionist's data and lesson log, in one fillable PDF: a group roster, a five-session lesson plan and log with a data-point tick on every session, one data sheet for each reading skill (phonemic awareness, phonics, sight words, fluency, vocabulary and comprehension, running records), a fluency graph scaled to words correct per minute, a weekly progress summary with the decision rules printed on the page, a change log, a parent note, an attendance grid, an exit and move decision sheet, and a filled-in example. What This Is, And What It Is Not
It is a fillable PDF. Open it in a PDF reader, click a line, a box or a cell, type, press Tab to move on, and Save As once per group or per student. It works offline with no account or subscription. It is NOT a Google Sheets file, a Google Doc or an Excel workbook, and it does not calculate percents or draw the graph.
It is not a program, a curriculum, or a set of probes (the probe forms the student reads from are a separate product, IEP Goals and Progress Monitoring Probes: Reading and Math). It names no program and asserts no benchmark, norm or threshold: every place a threshold would go says "as your program specifies" and gives you a blank. Tiers are written the way your school labels them. The exit sheet records the team's decision against the line the team wrote; it does not make the decision.
Who This Is For
Reading interventionists, Title I and MTSS teachers, and resource teachers running small reading groups in kindergarten through fifth grade who need the same data point taken the same way every session, a Friday summary that fits on one page, and a change log that explains the graph in May.
