The five sheets a counseling goal is actually measured on, free, print and go: an individual session sheet, a group session sheet for up to six students, a daily check-in / check-out log, a skill practice trial sheet for goals written as "in __ of __ opportunities", and a coping strategy use log.
WHAT IS INCLUDED - 9 pages, 5 sheets
- Individual Session Data Sheet - 8 goal rows across 6 columns (goal or target, skill practiced as the plan says, a 0-10 rating, trials + or -, prompt level, notes), a header for student, date, length and setting code, and three boxes at the foot for activity and materials, the student's response in one line, and next session
- Group Session Data Sheet - up to 6 students on one page: goal, a 0-10 participation rating, skill shown + or -, prompt level, and what the student did in a few words, with a header for the skill practiced today and a second 6-row table for one note per student
- Daily Check-In / Check-Out Log - 12 rows for the students you see at arrival and dismissal: date, student, morning rating, afternoon rating, goal met Y or N, adult initials, and one line on what was different today, plus a box for who was out and who covered
- Skill Practice Trial Sheet - 8 target-skill rows, 10 chances marked + or - across, with prompt, count and percent columns, a header for the criterion from the IEP, and a box for how the chances were set up
- Coping Strategy Use Log - 11 rows: date, strategy used, the situation it was used in, prompted (V / M) or independent (I), the outcome in observable words, and initials, with a box for the situations where the strategy is hardest to use
- How to Use These Five Pages - a table matching the goal's wording to the sheet, three numbered steps, the 7 marks all five sheets use (+, -, I, V, M, 0, X), a blank 4-band rating-anchor table you fill in, and the 5 setting codes (IND, GRP, PI, CI, CON)

What This Is Not
Not a counseling curriculum, not a lesson bank, not an assessment, and not a crisis plan. Nothing here says which strategy to teach, how to respond to a student, or what a rating should be: the anchors, the strategies and the criteria are yours to write in from the IEP or the plan. It is a print PDF: nothing here calculates the percent, and it is not a Google Sheets file or a spreadsheet.
