About this resource
The rating gets written down. That is the whole job of this file: the anchors defined before the first session, the criterion at the top of every sheet, the rating or percent onto the tracker the same day, and the missed session logged the day it was missed.
This is the clipboard for a school counselor, school social worker or school psychologist who delivers counseling as a related service on an IEP, and who usually runs the check-in / check-out program too, as a fillable PDF. Type the roster, the schedule, the anchors and the sheet headers once, Save As per student, and pen the ratings during the session. Every line, box and cell is a type-in field; on a tablet the whole thing can be typed.

Setup
- Which Sheet for Which Goal - seven goal wordings matched to the sheet that measures them, and a table of how the paper moves each session, each day, each week and each quarter
- How the Type-In Fields Work - one page, so nobody emails you
- Code Key and Rating Anchors - four rating bands on a 0 to 10 scale you define per goal, seven skill-practice and prompt codes, five status codes, five attendance codes with a column for your district's make-up rule, and five setting codes (individual, group, push-in, check-in, consultation)
- Caseload Roster - 15 rows: goal areas, minutes per the IEP, individual or group, which sheet, IEP date, review date

- Individual Session Data Sheet - 8 goal rows: skill practiced, rating 0 to 10 or trials + / -, prompt level, notes; three cards for activity, response and next session
- Group Session Data Sheet - 6 students: goal, participation rating, skill shown, prompt; then one note line per student
- Daily Check-In / Check-Out Log - 12 rows: date, student, morning rating, afternoon rating, goal met, adult initials, one line

Tracking And Reporting
- Cumulative Tracker - 36 sessions per goal in three blocks: date, setting, rating or percent, with the three-in-a-row rules printed on the page
- Progress Graph - twelve sessions, hand-plotted, aim line first; a rating is plotted at ten times its value
- Session Notes - two per page: targeted and the data, response, skill practiced, plan
- Quarterly Progress Summary - 6 goal rows with baseline, current level from the tracker, status code, one sentence; minutes owed, delivered, made up and still owed
- Family Note - what we worked on in family words, something that went well, one thing to try at home, and a bottom half the family sends back
- A filled-in example for an invented fifth grader: eight rated sessions on the tracker, a check-in week, the quarterly summary row and a session note, so you can see the status and the next step come out of the numbers
What This Is, And What It Is Not
It is a fillable PDF. Open it in a PDF reader, click a line, a box or a cell, type, press Tab to move on, and Save As once per student. It works offline with no account or subscription. It is NOT a Google Sheets file, a Google Doc or an Excel workbook, and it does not calculate averages or draw the graph. Prefer paper? Print it blank.
It is not a counseling curriculum, a lesson bank, an assessment or a crisis plan. Nothing in it says which strategy to teach, how to respond to a student, or what a rating should be; you write those in from the IEP or the plan and your own judgment. The logs record what happened and what was done as the plan says. Whether a contact or an incident must also go on a district form is your district's rule, and the contact log leaves a line for it.
