About this resource
A case manager's week does not fit in a teacher planner built for one class and one gradebook. This is the planner for the person with twelve students, three meetings, a data sheet per student and a phone that rings: undated, so you type the month names and the dates and it never expires, in one fillable PDF. Year at a glance, a caseload snapshot, two undated month calendars, a deadlines and meetings list, two weekly layouts, a daily plan two per page, a group lesson plan, a meeting-prep week, a data-collection week, to-do by category, a quick communication log, substitute notes for the week, a professional-learning log, a supplies tracker, a month reflection, year-end hand-off notes, and one October week filled in for an invented case manager. Sunday takes twenty minutes; the month feeds the week and the week feeds the day.
WHAT IS INCLUDED - 24 pages, 1313 type-in fields
Setup
- How This Planner Is Laid Out - which page for which habit and when, the Sunday twenty minutes in four steps, how the year page becomes the month, the week and the day, and a twelve-month table of what often lands in a case manager's year with a blank column for your district's own dates
- How the Type-In Fields Work - one page, so nobody emails you
- Year at a Glance - 12 months you name, by 5 kinds of date: IEPs due, evaluations due, progress reports, meetings, other
- Caseload Snapshot for the Planner - 12 rows: student, grade, IEP date, next progress report, services in a few words, one line of what matters this year
Month
- Monthly Calendar, undated - 6 weeks by 7 days of large day cells, each with a short date-corner row you type the date into, a "This month" side list and a no-school-days box
- Monthly Calendar, second copy - the same grid, so this month and next month can both be open
- Monthly Deadlines and Meetings List - 20 rows in date order: date, student, what is due or what the meeting is, kind, done; the page Sunday copies the week off
Week And Day
- Weekly Plan - Monday to Friday across, morning, midday and afternoon blocks down, a date row, a must-do list and a waiting-on list of 8 lines each
- Weekly Plan, Hourly - 7:00 to 4:00 in 19 half-hour rows across five days, for the week with three meetings and a testing session
- Daily Plan - two days per page, each with a 7-block schedule (time, students or group, what, data to take), three priorities, calls to make, notes carried to tomorrow, and a cut-or-fold line
- Lesson and Group Plan - 4 groups: who is in it, objective in the goal's words, materials, activity, data to take, notes for next time
- Meeting Prep Week View - 6 meetings: date and time, student, meeting, what to bring, who to remind, done; a what-to-bring-every-time checklist and a who-has-not-confirmed box
- Data Collection Week View - 12 students by Monday to Friday, with the sheet each student is on this week and an all-five column
Month And Year End
- Month Reflection - what went well, what to change (one change, not five), students to watch, one thing to celebrate, and what carries into next month's list
- Year-End Hand-Off Notes - 12 rows for whoever opens the caseload in August: first name, next grade or school, what worked, what to keep an eye on, open items; no dates, scores or contacts
- A filled example - an invented October for Ms. Ortiz and six students: the deadlines list (9 rows, 4 in the week, 2 of them meetings) and the week of Oct 13 copied off it, with both meetings in their day's block and the must-do and waiting-on lists filled
What This Is, And What It Is Not
It is a fillable PDF. Open it in a PDF reader, click a line, a box or a cell, type, press Tab to move on, and Save As once per month or per week. It works offline with no account or subscription. It is NOT a Google Sheets file, a Google Doc or an Excel workbook, and it does not know what month it is or move a date for you. It is undated on purpose: you type the month names and the dates, so the same file works this year and next.
It is not the Case Manager Binder and does not repeat it: there is no IEP or evaluation tracker, no file audit, no meeting agenda, no student information sheet and no service log here; the binder and the service log carry those, and this planner's dates are copied from them. Nothing on these pages states a timeline or a due-date rule; the year table on page 3 has a blank column for your district's dates, and the year page is filled from the IEPs.
Who This Is For
Special education teachers and case managers who plan by the week and are tired of a planner built for one class, resource and self-contained teachers with groups across a day, related-service providers who want the same undated month and week pages, and anyone whose Sunday planning has to start from a deadlines list rather than a lesson.
