About this resource
You see twelve students on eight different days, someone is out, a school event takes Thursday, and by March you cannot say how many sessions you owe. This puts the whole year of session attendance on paper in one file: a calendar for every month, a log, a caseload roll-up and a make-up tracker, all fillable.
EVERYTHING INSIDE — 28 pages
- 12 monthly calendars, August 2026 through July 2027: Monday to Friday, a type-in box on every weekday (260 boxes in all), and a count strip under each month
- Setup and codes: your terms, the days your school has no sessions, and five codes (P present, A absent, M make-up held, C canceled by me, N no school)
- A weekly caseload sheet: 18 rows of students or groups by Monday to Friday
- A session log for one student: 17 lines with date, code, time, individual or group, and what you worked on
- A caseload attendance roll-up: 17 student rows with planned, present, absent, make-up, canceled, no school, held and open
- A year roll-up: 12 month rows (August to July) and a year total
- A make-up tracker: 15 rows with the missed date, reason as told, make-up date and done
- A note home to families with blanks in your own words
- A heads-up page for classroom teachers: times, where, and what helps
- A phrase bank of 32 starters in 8 groups
- A filled example: one invented student's September 2026 calendar and a three-student roll-up, every total re-added from its own row
- A binder cover, a spine label and 8 cut-apart tabs
- Start Here, a contents page with page thumbnails, and a "what's next" back page
Most attendance sheets are one grid for a class. This one is built for the way a speech caseload runs: one calendar a student or group, the week across the whole caseload, and the make-ups you still owe.
What This Is, And What It Is Not
It is a fillable PDF. Type into the boxes in Adobe Acrobat Reader (free) or Preview, or print and write by hand. It works offline with no account. It is not a spreadsheet or an app and does not add anything up for you. No holiday, break or school date is printed: those come from your school, so each is a blank. It does not say how many sessions a student is owed, when a miss is made up or what a miss means; your school's rule gets a blank line on two pages. The students and the SLP in the example are invented and the page says so.
Who This Is For
Speech-language pathologists, school-based or itinerant, who want their session attendance in one place. The teacher heads-up page and the note home are for the adults around the caseload.
