About this resource
The number gets written down. That is the whole job of this file: the criterion at the top of every sheet, the percent onto the tracker the same day, the missed session logged the day it was missed.
This is the clipboard for a school SLP, as a fillable PDF. Type the roster, the schedule and the sheet headers once, Save As per student, and pen the trials during the session. Every line, box and cell is a type-in field; on a tablet the whole thing can be typed.
WHAT IS INCLUDED - 22 pages, fillable, 1,500+ fields
Setup
- Which Sheet for Which Goal - match the goal's wording to a sheet once, in September.
- Code Key and Cue Levels - trial marks, six cue codes, five status codes and seven articulation levels, each with a line to define it the way your practice does.
- Caseload Roster - goal areas, minutes per week per the IEP, group, sheet, IEP and re-evaluation dates.
- Weekly Therapy Schedule - blocks by days, push-in or pull-out.
Attendance
- Attendance and Make-Up Log - one row per student per month: sessions owed per the IEP, eight date cells, what was missed and why, when it was made up.
Six Data Sheets
- Individual Session (seven goals, cue, ten trials, count, percent, notes)
- Articulation (sound, position, level, criterion; nine dated rows; a level column so a drop in percent reads as a level change)
- Articulation Word Tally (twenty target words, five trials each)
- Language Trials (receptive or expressive targets, cue, ten trials)
- Group Session (four students, two goals each, one note per student)
- Rating Scale (for pragmatics, fluency strategies, voice, carryover: define the four levels first, then rate eight sessions)
Tracking And Reporting
- Cumulative Percent Tracker - 36 sessions per goal with the level or cue under every date.
- Progress Graph - hand-plotted, aim line first.
- Session Notes - two per page: targeted and the data, response, materials, plan.
- Quarterly Progress Summary - each goal with baseline, current level from the tracker, status code, one sentence; plus sessions owed, delivered, made up and still owed.
Home And Classroom
- Speech Home Note - in family words, one thing to practice this week, a practiced-this-week row.
- Classroom Carryover Check-In - a week with the classroom teacher: heard the target, where, what it sounded like, prompted or not.
Plus an invented kindergartener's articulation sheet, October attendance row, quarterly summary row and a session note, filled in, so you can see how the level column and the criterion do the deciding.
What This Is, And What It Is Not
It is a fillable PDF. Open it in a PDF reader, click a line, a box or a cell, type, press Tab to move on, and Save As once per student. It works offline with no account or subscription. It is NOT a Google Sheets file, a Google Doc, or an Excel workbook; it does not calculate percents or draw the graph. Prefer paper? Print it blank.
It is not a therapy program, not word lists, not goals, and not a screener. Nothing in it says which cue to use, when to move up a level, or what counts as mastery; you write those in from the IEP and your own judgment. Whether therapist-out sessions must be made up is your district's rule, and the make-up log leaves a line for it.
Who This Is For
School-based SLPs and SLPAs, and the special education teachers and paraprofessionals who collect communication data between sessions.
