About this resource

Everything inside: 23 pages · four timed student practice pages (16 items each, in three labeled sections) · a shared scoring-keys page for all four forms, with a worked example of the most common error · a data log and graph scored as problems correct per minute · a quick-response observation checklist · a caseload summary · a quick reference card for a second adult · a family update · a 19-starter phrase bank · a binder cover and 4 cut-out divider tabs · a worked, invented example.

This store already has one UNTIMED square roots and perfect squares probe (ten items a form, right or wrong, no clock). Nothing in the store times this skill, and a paid timed-drill listing for this exact skill already carries real reviews, showing the demand for a timed version. This file is that second measure, the same split CBM-Math progress monitoring draws everywhere else: can the student recognize, root and sequence perfect squares, and separately, how fast and accurately can the student do it under a clock. Four equivalent timed forms, sixteen items each, in three labeled sections so the task is stated once per section instead of once per item — eight items asking "Is it a perfect square?" (Yes or No, four genuine perfect squares and four near-miss non-squares mixed together), four asking for the square root of a perfect square (always a whole number — no item anywhere asks for an approximate or irrational root), and four asking for the next perfect square after one shown — answered in writing under a time limit your team sets, scored whole-item: an answer either matches the key exactly or it does not, because every item is a single retrieved-or-computed value with no running carry the way a written algorithm has. The score is problems correct per minute (PCPM) and accuracy together, with a second page that watches HOW the answer was reached (retrieved directly, worked out by counting or a times table, or guessed/blank) and gives the team a place to write its own call on where practice should go next, never a cutoff this file imposes.

What This Is, And What It Is Not

It is a fillable PDF. Open it in a PDF reader, click a field, type, Save As once per student. It works offline with no account or subscription. It is NOT a Google Sheets file, a Google Doc, or a PowerPoint deck, and it is not a norm-referenced test — it carries no fixed fluency benchmark score anywhere in the file. The four student practice pages and the scoring-keys page print; the goal page, log, checklist, caseload page, second-adult reference and organizer are type-in.

It is not the store's existing Square Roots and Perfect Squares Progress Monitoring Probes | IEP Data Sheet, which is untimed — ten items a form, no clock, right or wrong. Use that file to check whether a student can recognize, root and sequence perfect squares at all; use this one once the skill is solid, to see whether it has become fast and automatic. Every root and next-perfect-square item in this file uses a perfect square only; no item anywhere asks for the irrational root of a non-perfect square or a decimal approximation — that is a different skill (Classifying Rational and Irrational Numbers).

It is not a curriculum, and it is not a rule about how many problems per minute counts as "fluent." What fluent enough means for a given student is the team's own judgment, written in from the baseline and the IEP; this file never sets that number on its own — the quick-response checklist page is a blank the team fills in, by design. No age, eligibility or IDEA-timeline claim anywhere; no health data is pre-filled, and the example student (Amara) is invented and named as such on page 22. Because the domain is only 13 perfect squares, the same square can come back on a later form under a different task — disclosed here, not hidden: no item repeats within one form, and no two forms are identical.

Who This Is For

Math interventionists, special education teachers and case managers who need a timed, repeatable way to track square-root and perfect-square fluency — not just whether the student can do it, but how fast and accurately it runs — plus a second, condensed page for whoever else gives the probe.

If You Want More

Square Roots and Perfect Squares Progress Monitoring Probes | IEP Data Sheet is the untimed, accuracy-only companion for the same skill. Exponents and Powers of Ten Fluency, Timed Progress Monitoring Probes is this file's own goal-log-graph engine, applied one skill tier later in the same compute-a-number cluster. Order of Operations Fluency, Timed Progress Monitoring Probes is a second timed computation-fluency probe at the same late-elementary/middle grade band. Progress Monitoring Tracking Sheet | IEP Goal Tracking Data Collection Editable is the weekly summary and progress-report writer these graphs feed.
