About this resource
EVERYTHING INSIDE — 25 pages, 263 type-in fields
- A 10-square sticker chart: a name line, ten big squares with a sticker spot in each, and a MY REWARD box for a picture goal
- A 20-square sticker chart, the same layout for a longer goal
- A 30-square sticker chart for a longer goal still
- My Own Goal Chart: the student draws the goal, writes the numbers and fills the squares
- Two desk-size 10-square charts on one sheet, for a folder or a second student
- 12 reward pictures, 2 inches square, in full color: crayons, paint, kite, bike, slide, game, music, balloon, teddy bear, gift, ice cream, bubbles
- A strip of 24 small reward pictures (two of each), to cut and glue at the end of a chart
- 12 blank reward cards with a type-in word line, for any reward the printed pictures do not cover
- The same 12 reward pictures in black-and-white ink-saver, for the photocopier
- A how-to page: before the first day, the three moves every time, changing the reward, starting over
- A Sticker Log (20 rows: date, square, what the sticker was for in your own words, chart full?, reward given, initials)
- A weekly roll-up (8 weeks) with a stickers-per-week graph
- A filled example: an invented student, one 10-square chart on the log and three weeks on the roll-up
- A family page with a small chart and six home reward pictures on one sheet
- A hand-off page so a paraprofessional or another classroom runs the same chart the same way
- A phrase bank: 23 lines in 6 groups (spoken lines for the chart, written starters for the log, the roll-up and the note home)
- A storage and binder label sheet: 11 labels and a spine strip
- Start Here, a contents page with page thumbnails, and a "what's next" back page
The Charts
Every square is a rounded box with a small number in the corner and a faint dashed circle for the sticker. The student presses one sticker on each square, left to right, and the reward picture at the end is the goal. Student pages carry only a "My name" line, one short instruction with a picture, and big squares. Stickers are not included: use any sticker you already have.
The Reward Pictures
Twelve concrete pictures a young child can name in one word, in color and in a black-and-white set. Use them as a goal at the end of a chart, or pick your own and use a blank card.
What This Is, And What It Is Not
It is a fillable PDF: type into the name lines, the log, the roll-up, the family page, the hand-off page and the blank cards, or print and use a pen. It works offline with no account or subscription. It is NOT an editable online slideshow or word-processor document.
It is a way to see progress toward a picture goal and to write down what happened. It is not a behavior plan or a program, and it promises no outcome. What earns a sticker, and the number that counts as enough, are blanks for your team and your district's own wording.
Who This Is For
Special education teachers and paraprofessionals who run a sticker chart for one student or a room, case managers who need it to show up as data, and the families who get the home page.
