About this resource
Strengths-based IEP teams need a place to systematically record what a student does well, is interested in, and is proud of — specifically enough to build a goal from, not just a courtesy paragraph. This fillable PDF is that place: a strength and talent checklist across six domains, asset mapping, an observation checklist, family and student input in their own words, a worksheet that turns one identified strength into usable language, a connection guide for the meeting where goals get drafted, a snapshot summary, and a complete worked example. Type the header fields once, Save As per student, write the evidence.
WHAT IS INCLUDED - 18 pages, 387 type-in fields
Setup
- What This Profile Documents - five jobs, each mapped to the page that does it, a table of how the paper moves through a term, and who typically fills in each part
- How the Type-In Fields Work - one page, so nobody emails you
- Strengths Caseload Roster - 13 rows: student, grade, primary strength or domain, key interest, documented by, updated
The Checklist And The Map
- Strength & Talent Checklist, Academic / Social / Creative - 18 indicator rows (6 per domain), a check column and an evidence column
- Strength & Talent Checklist, Physical / Leadership / Interests - 18 more indicator rows across the remaining three domains
- Asset Mapping by Domain - one row per domain (6 rows): does well, enjoys or chooses, is proud of
- Strengths Observation Checklist - 10 rows: date and time, setting, domain, what happened in observable words, observed by
Voice, Language And Use
- Family and Student Strengths Input - separate spaces for the family's and the student's own words, a paragraph for the draft, and where it shows up later
- Capability-Statement Worksheet - 8 rows: strength identified, evidence (source and date), descriptive capability statement
- Strength-Led Present-Level Frame - six short sentence frames, one per domain
- Connection Guide: Using a Strength When a Team Writes a Goal - 6 rows, one per domain, of questions the team might ask, and a notes column
- Strengths Snapshot Summary - top strengths, key interests, how a strength could connect to a goal, the family's and student's words in one line each, a shared-with table, and a one-sentence present-levels opener
- Self-Check Before It Goes in the Draft - 12 questions
- A filled-in example for an invented fourth grader: the checklist counts, the asset map entry, the capability statement, the family's words, and the strength-led paragraph they add up to
- Strengths Word Bank - 12 pairs, the vague compliment next to specific, evidence-based language
What This Is, And What It Is Not
It is a fillable PDF. Open it in a PDF reader, click a line, a box or a cell, type, press Tab to move on, and Save As once per student. It works offline with no account or subscription. It is NOT a Google Sheets file, a Google Doc, or an Excel workbook.
It is not a legal template, and it does not determine whether a student qualifies for services — that is your team's own process. It does not write a goal for you: the connection guide is a set of questions to ask before a goal is drafted, not a script that writes one. The capability-statement worksheet turns an observed strength into plain, descriptive language; it is never a legal or eligibility statement.
Who This Is For
Special education teachers and case managers building a strengths-based present level, related-service providers documenting a strength from their own sessions, and teams who want the family's and the student's own words in the file before the meeting where goals get drafted. Pairs with the Present Levels (PLAAFP) Writing Kit, which turns the strength-led paragraph from page 16 of this kit into the full present-levels section, and with IEP At A Glance Editable, the one-page snapshot the strengths summary on page 14 feeds.
