What This Is, And What It Is Not
It is a fillable PDF. Open it in a PDF reader, click a field, type, press Tab to move on, Save As once per student. It works offline with no account or subscription. It is NOT a Google Sheets file, a Google Doc, or a slide deck.
It is not a scoring or diagnostic instrument, and it does not decide anything on its own. Every plain-language goal restatement and every ‘I can’ statement is written IN by the team, from data already collected — never generated or inferred by this file. It is not a rule about who is told what, when, or how a school discusses any label or category with a student or family; that stays entirely the team’s and the district’s own decision. No age, eligibility, or timeline claim anywhere. No health data is pre-filled, and the example student (Micah) is invented and named as such on the example page.

Case managers, special education teachers, and related-service providers who want a student to be able to talk about their own progress in plain language — for ongoing documentation between meetings, not meeting-day prep alone.
