About this resource
Student-centered IEP practice asks for more than a student who shows up to a meeting once a year — it asks for a student who has an ongoing sense of how their own goals are going, and some words for why. This file is that: a private, weekly rating the student marks themselves (better, about the same, or harder), open reflection prompts on what helped and what was hard, a strategy log in the student's own words, a wish-list page for what to work on next, the same check-in done at home, and a roll-up that turns weeks of marks into one page for a meeting or a report.
This is NOT the Student-Led IEP Meeting Toolkit (meeting-prep: an agenda, a script, a rubric, aimed at one day) and NOT the Student Self Monitoring Checklists (an in-the-moment behavior tool: a box checked against what a teacher just saw). It is a periodic, private self-assessment tool — the gap between those two.
EVERYTHING INSIDE — 21 pages, 10 working pages, a filled example
- A goal-progress rating log (11 rows): date, goal (short name, teacher types it), a three-point self-mark — BETTER / SAME / HARDER — and a one-word-why column
- A goal-difficulty self-report (12 rows): the same log shape, marked TOO EASY / JUST RIGHT / TOO HARD
- Reflection prompts (3 prompts): What helped me learn this? What was hard about this? What will I try next?
- A strategy-reflection log (12 rows): the strategy tried, whether it helped (YES / SORT OF / NO), and a note in the student's own words
- A celebration card: six prompts (something I'm proud of, who helped, how it felt, what I want to remember, someone to tell, a word that sums it up) plus a ten-mark strip for repeat use
- A "next goal" wish page: six prompts (something to get better at, why, what might help, who can help, what it could look like in a few months, the first small step) plus a team-response line
- A home check-in (12 rows) — the identical three-point rating log, done at home with a family member: the second setting this file is built for
- A quarterly reflection summary (8 rows) that counts the marks already made on the rating log by hand, per goal
- A phrase bank: 23 sentence starters across 5 groups (how a goal is going, what helped/was hard, being proud and asking for what I need, the quarterly summary line, the family/team update)

What This Is, And What It Is Not
It is a fillable PDF: type into the log, the prompts and the tables, or print and use a pen. The folder cover, spine labels and divider tabs print like any other page and are meant to be cut apart. It works offline with no account or subscription. It is NOT a Google Sheets file, a Google Doc, or a word-processed file, and it does not add up the counts on the quarterly page for you.
A self-rating describes how ONE goal felt to the student that day — never a mastery rule, a passing score, or a determination about the student. Nothing on any student-facing page names a category, a label, or a clinical finding; those calls stay with the team, using its own judgment, in a conversation this file can only help start.
