About this resource

Portfolio-based assessment only works if the work itself is dated, described and linked to something — a skill, a goal, a level of support. Most teams end up with a folder of loose papers and no system for turning it into meeting evidence. This file is that system: log an artifact once, mark how much support it took, tie it to a goal, roll the quarter up, and choose what actually goes to the meeting — set up once, filled all year, and built to drop straight into a binder.

What This Is, And What It Is Not

It is a fillable PDF: type into the tables, the reflection prompts and the communication pages, or print and use a pen. The binder cover, spine labels and divider tabs print like any other page and are meant to be cut apart. It works offline with no account or subscription. It is NOT an editable online slideshow or word-processor document, and it does not grade, score, or set a mastery rule for any artifact.

The support-level mark (Independent / with a Cue / Direct Help) describes what was seen on ONE piece of work — never a rule about the student, and never a mastery or eligibility determination. Whether an artifact shows a goal is met is a decision for the team, using its own criteria, in the blanks this file leaves for it.
