About this resource
It is 3:15, the bus bell is ringing, and you are writing down the whole day on the back of a worksheet. This gives a substitute one place to type or write it: the day at a glance, notes by period or time block, attendance, the work that is done and the work that is not, and a note the teacher finds first.
EVERYTHING INSIDE — 20 pages
- A one-page day report: how the day went, what went well, what needs attention, students to thank, students to check in with, messages and where the work was left
- Notes by period for middle and high school: 8 period rows with Class or Subject, Here / On List, What We Did, Where We Stopped and Notes
- Notes by time block for one class all day: 9 rows with Time, Block, What We Did, Finished? and Notes
- Attendance for the day: 24 student rows with Here, Absent, Tardy, Left Early and a note, plus a count
- Who left and came back: 13 rows with Left At, Went To, Back At and Notes
- Students to thank (7 rows) and students to check in with (5 rows), with a blank line for your school's rule on what happens next
- Work finished and work left over: 12 assignment rows and a roll-up for the day
- Room, supplies and messages: eight boxes for supplies, things moved, lost and found, technology, office messages, visitors, family notes and schedule changes
- My Day With the Sub: a page a child fills in, with a big My name line, a drawing box, three faces to circle and a writing box
- A Thank-You Card for Our Sub: a page a child fills in, with a My name line, ruled writing, a drawing box and a From line
- A note to the returning teacher with blanks in your own words
- A sub days log with a roll-up: 14 rows, for a teacher to keep a year of sub days and count them
- A phrase bank of 32 starters in 8 groups
- A filled example: one invented grade 3 class and one day, with the attendance count and the assignment count re-added from their own rows
- A binder cover, a spine label and 8 cut-apart tabs
- Start Here, a contents page with page thumbnails, and a "what's next" back page
Most sub report forms are one page of lines. This one gives the sub a page for the whole day, a version for periods and a version for time blocks, the attendance, the work, the thank-yous and the note, and gives the teacher a way to keep a year of sub days.
What This Is, And What It Is Not
It is a fillable PDF: type into the pages, or print and use a pen. It works offline with no account or subscription. It is NOT a Google Slides file, a Google Doc or an Excel workbook, and you cannot drag the boxes around or restyle the pages.
It is the working paper a substitute leaves behind. It is not a lesson plan and it does not say what a school should do about a student. That is the teacher's and the school's rule, and it gets a blank line. No health information is filled in. The example class, teacher and students are invented and say so on the page. The only cutting is the 8 binder tabs on the second to last page.
Pictures are Google Noto Emoji (Apache License 2.0), credited on the last page.
Also In The Store
The plan a teacher leaves for the sub is the Sub Lesson Plan Template, class information for the sub is in the Sub Folder, a full binder is the Substitute Teacher Binder, and a running log of notes is the Substitute Teacher Communication Log. This file is what the sub writes back.
Who This Is For
Substitute teachers, and classroom teachers, special education teachers and paraprofessionals in kindergarten through 12th grade who want a clear report waiting when they walk back in.
