About this resource
Every task analysis layout a chained routine needs, in one fillable PDF, and nothing else: 12 steps by 8 sessions, 6 steps by 12 sessions, 20 steps by 5 sessions, a chaining tracker that turns one row per step into a sessions-to-independent count, the daily living routine checklist, a prompt code key with a step-writing guide, and a filled example. Type the routine and the code key once, Save As per student, pen the prompt codes.
WHAT IS INCLUDED - 12 pages, 654 type-in fields
Setup
- Which Layout for Which Routine - a chooser that matches the routine to the sheet that fits it, and a table of how the paper moves each session and each interval
- How the Type-In Fields Work - one page, so nobody emails you
- Prompt Code Key and Step-Writing Guide - six prompt codes from independent to full physical, edited to match your team's hierarchy, three rules for turning a routine into steps, and a twelve-line block to draft them
Three Task Analysis Layouts
- Task Analysis Data Sheet, 12 Steps by 8 Sessions - twelve step rows, eight session columns, the bottom rows count the I's
- Task Analysis Data Sheet, 6 Steps by 12 Sessions - six step rows, twelve session columns, for a short routine probed often
- Task Analysis Data Sheet, 20 Steps by 5 Sessions - twenty compact step rows, five session columns, for a routine a narrower grid would crowd
Tracking And Daily Use
- Chaining Tracker - twelve step rows, not a session across the columns: date taught, prompt at start, sessions to independent, date independent, a maintenance check
- Daily Living Routine Checklist - two routines a page, seven steps each, five days across, the steps typed by you
- A filled-in example for an invented fifth grader: a six-step, twelve-session lunch routine, with the steps-independent and percent rows computed from the sessions above them, and the chaining tracker rows those same sessions produce
What This Is, And What It Is Not
It is a fillable PDF. Open it in a PDF reader, click a line, a box or a cell, type, press Tab to move on, and Save As once per student. It works offline with no account or subscription. It is NOT a Google Sheets file, a Google Doc or an Excel workbook, and it does not count the I's in a column for you.
It is not a curriculum, not a list of routines, and not a prompting procedure. Nothing here says which prompt to use, when to fade it, or what a student should be doing at any age; the team writes the prompt hierarchy in, from the IEP and its own judgment. The steps of every routine are yours to write; this file only records the prompt level.
Who This Is For
Special education and life-skills teachers, job coaches and paraprofessionals who collect chained-routine data for IEP goals, and any team that wants the twelve-step grid for one routine and the six-step grid for another without buying a full life-skills binder to get them.
