About this resource
The ongoing data-collection companion to a transition plan: not a second copy of one. Track work-experience hours and tasks, trial job-readiness skills across sessions, record community-based instruction (CBI) outings, check postsecondary goal status across three areas (education/training, employment, independent living), log employer and site contacts, check generalization across three different settings, plot progress on a tracker and graph, write the quarterly summary from the data, pull together the meeting-ready snapshot, send home a family update note. Every entry is dated and coded; the file does not predict what a student will do. Type the roster, the sites, the codes and the headers once; Save As per student; pen the hours and codes during visits and outings.
WHAT IS INCLUDED — 19 pages, 943 type-in fields
Setup
- Which Sheet for Which Transition Data Need — a chooser table matching eight data-collection needs to the sheet that carries them, and a "How the Paper Moves" table showing the weekly and checkpoint workflow
- How the Type-In Fields Work — one page, so nobody emails you
- Code Key — independence/prompt hierarchy (six levels: I, V, G, M, PP, FP), work behavior rating (1 to 4), postsecondary goal status codes, and settings (school, home, community, worksite)

Tracking, Reporting And Home
- Cumulative Tracker — one skill or goal, 36 weeks across three blocks of 12; write week number, hours/count/percent, and independence code from each data sheet

- Quarterly Progress Summary — one student, one reporting period; six goals with baseline, current status from the tracker, status code, and one sentence per goal
- Meeting-Ready Transition Data Summary — one page for the annual review: hours and sites this year with independence trend, postsecondary goal status at a glance, questions for the team, student's own words, opener sentence, sites to thank, and next steps
- Home / Family Update Note — what we did at work or in the community this week in family words, one thing to try at home, a half-page for the family to return
- Filled-in example for an invented grade-11 student (Maya) showing work experience hours (6.5 hours across 3 visits), job-readiness skill trials (four skills, eight sessions, three in a row at criterion by week 6), and postsecondary goal progress snapshots
What This Is, And What It Is Not
It is a fillable PDF. Open it in a PDF reader, click a field, type, press Tab to move on, Save As once per student. It works offline with no account or subscription. It is NOT a Google Sheets file, a Google Doc or a PowerPoint presentation.
It is not the Transition IEP Planning Kit: this file does not draft a postsecondary goal, write a course of study, or prepare a student-led IEP script—the planning kit does those, and the goals it produces are what this file's roster and progress pages track all year. It is not a job-coaching procedure or a certification of what a student can or will do. Every independence code, work-behavior rating and goal-status mark is written in from something observed and dated; the file never predicts an outcome. It is not a curriculum. No age, eligibility, diploma or IDEA-timeline claim anywhere; your district's rule on CBI supervision, ratios, and consent gets a blank line to write it on.
