About this resource
"Will pursue a career in a field of interest" cannot be checked. "Will work part time as a service technician's assistant at an independent repair shop while enrolled in the automotive program" can.
This kit builds the transition plan from the student outward, as a fillable PDF: what the student wants and can do now, postsecondary goals in words somebody could check two years after exit, the courses and services that get there, and the student's own script for the meeting. Type it once, Save As per student; the student and family pages print blank and go out.

From The Student
- Student Interview and Preferences - interests, strengths, what they want to learn, do and where they want to live after school, what is hard and what helps, in their words.
- Family Input for Transition - what the family sees, what worries them, what the student already does alone, what they want to learn more about.
- Transition Assessment Summary - every formal and informal assessment, dated, with the result in observable terms and the goal area it points toward.
The Goals And The Path
- Postsecondary Goal Frames - education or training, employment, and independent living where the team decides it is needed, each with a frame and a four-point check.
- Course of Study Planner - grades 9 through 12 and extended, courses and experiences, each naming the goal it serves; diploma and credit rules written in as your district defines them.
- Transition Services and Activities - eleven rows: what, type, provider, when, which goal.
- Annual Goal Alignment - the annual IEP goals that build the path's skills, each pointing at a postsecondary goal, with baseline and measurement.
- Agency Linkages and Community Resources - with consent and contact recorded as your district's process requires.
Two Inventories
- Independent Living Skills (16 skills in five areas) and Employment Readiness (14 skills in four areas), each rated 1-4 by the student and by an adult, with evidence, and room for what to build this year.
The Student'S Meeting And The Exit
- Student-Led IEP Preparation - their disability in their words, the accommodations they actually use, their goals, what they want to ask for, the opening script, and the signal if they get stuck.
- Exit Planning: What Leaves With the Student - nine items, who prepares each, when the student receives it.
- Four-Year Transition Timeline - six rows by five columns, kept on top of the file.
Plus the goals and path FILLED IN for an invented eleventh grader, with the paragraph from his interview that all of it came from, and a page of ten postsecondary goals written in a hurry beside the same ten written so they could be checked.
