About this resource
Everything a team needs to track a twice-exceptional student's goals on BOTH clusters — the advanced-ability or gifted cluster and the disability or support cluster — without either one disappearing behind the other, in one fillable PDF.
EVERYTHING INSIDE — 24 pages, 576 type-in fields
- A binder cover, a spine label and 6 divider tab labels (cut-and-tape organizer set) with a table of what goes behind each tab
- A dual profile card holding the strength cluster and the challenge cluster side by side, plus a sources-of-evidence checklist
- A school-context profile: 8 settings (whole-group instruction through unstructured time), what each cluster looks like in each, and the support already in place
- A chooser that matches what you are tracking to the page that fits it (7 rows)
- A strength-area goal tracker with a 7-row milestone table, and its own 13-row data log
- A challenge-area goal tracker with an 8-row benchmark table, and its own 13-row data log
- A quick daily dual check-in (one week, both clusters, about two minutes a day) with a rollup row
- A 2e caseload roster (13 rows) for the whole caseload, both clusters, at a glance
- A dual-cluster progress graph — two lines, two aim lines, on one page
- A quarterly summary that reports each cluster separately, never averaged into one number
- A general education classroom observation form — the second audience this file is built for
- A family update addressing both exceptionalities, ready to send
- A 23-phrase observable-language phrase bank across four groups (strength, challenge, how they interact, the quarterly-report sentence)
- A filled-in worked example on an invented sixth grader, two data logs and a quarterly note, every number traceable to its own table
What This Is, And What It Is Not
It is a fillable PDF. Open it in a PDF reader, click a line, a box or a cell, type, press Tab to move on, and Save As once per student. It works offline with no account or subscription. It is NOT a Google Sheets file, a Google Doc or an Excel workbook, and it does not calculate a rating or plot a point for you.
It does not determine whether a student is gifted or whether a student qualifies for a disability label — both are your team's own determination, made on your district's own paperwork and criteria. Every mark, rating and word on these pages is the team's own data entry, entered after that determination, never a scoring instrument that makes it. There is no clinical wording anywhere in the file, and the blank for your district's own reporting schedule stays blank.
Who This Is For
Case managers, gifted specialists and IEP teams working with a twice-exceptional student — gifted or advanced ability alongside a learning disability, ADHD or autism — who need one file that tracks both clusters without collapsing them into a single number, plus the general education teacher who sees the student every day.
