The store's accommodations line is built for one student at a time, tied to a plan on file. This file asks a different, classroom-wide question: are the three UDL principles — multiple means of representation, action and expression, and engagement — and basic physical, sensory, communication and cognitive access actually in place for every learner in the room, not just the ones with paperwork. A teacher or inclusion coordinator checks the room once a term, logs coverage weekly, tracks what individual students actually chose to use, writes down what still is not accessible, and rolls the month up into one page for a team meeting or a coaching cycle.

What This Is, And What It Is Not
It is a fillable PDF. Open it in a PDF reader, click a field, type, Save As once per term. It works offline with no account or subscription. It is NOT a Google Sheets file, a Google Doc, or an Excel workbook, and it does not calculate a percentage or score a classroom for you.
It is not the Accommodations and Modifications Checklist — the per-student menu a case manager checks off for one plan — and it does not replace it. It is also not the Accommodations Implementation Log, the gen-ed teacher's ongoing record of whether one student's plan actually happened. This file asks a classroom-wide question instead: are the UDL principles and basic accessibility features in place for every learner in the room, whether or not they carry a plan on file. A classroom can use this file AND either of those, for different purposes.
It is not a UDL training and not a set of prescribed activities. Nothing here tells a teacher which materials, technology or strategy to use, or decides whether a classroom "passes." No age, eligibility or timeline claim anywhere; no health data is pre-filled, and the example classroom and teacher (Ms. Alvarez) are invented and named as such on page 21.
