What This Is, And What It Is Not

It is not a vision test, and it is not a substitute for one. No page in this file names a condition, sets a passing score, or lists eligibility criteria. Every screening result is what the screener or nurse recorded, in a column that says so; every functional vision checklist row is a description of observed classroom behavior, never a reason for it. Whether a result or a set of observations means a student needs a formal evaluation is a decision for the school nurse, the family, and an eye care professional — this file only keeps the record.
