About this resource
Everything inside: 25 pages · three WH-question trial types (an oral listening trial, an independent reading check the student writes on, and a fixed 12-item quick spoken check) · a sample bank of six scenarios with every WH answer written out · a within-session frequency chart with a 4-session quick-trend table · a whole-caseload roster · a binder cover and 4 cut-out tabs · a 2-trial-type, 12-session tracker and graph · a quarterly summary broken out by WH type · a family update · a 19-starter phrase bank · a simplified paraprofessional form · an adult-only answer key for the independent check · a worked, invented example.
Speech-language pathologists, case managers and special educators are asked to show whether a WH-question comprehension goal — who, what, where, when, why, how — is changing, across many sessions, not just on a fixed set of scored forms. This file is the trial-data companion: run an oral listening trial, hand the student an independent reading check, or give the quick 12-item spoken check, mark whether the answer matched a detail actually stated in the text, add the session up on one chart, and roll it into a report and a family update that write themselves. Every mark says only whether a recorded answer matched the text — never a score against a published norm, an inferred leap, or a rule for when a referral is warranted.
WHAT IS INCLUDED — 25 pages
Set Up The Caseload
- WH-Question Comprehension Goal, Baseline and Rubric — what the goal measures in one paragraph, a sample goal with every bracket meant to be edited, the six-part goal frame as type-in fields, a 3-trial baseline table, a 2-row benchmark table, and the binary correct/incorrect rubric
- WH-Question Definitions, Marks and Conditions — the six WH types defined in one table, 4 response marks (+/−/SC/NR), 3 presentation-condition codes (read aloud once/twice, read silently), and a 4-item "before the first session" checklist
- Sample Scenario Bank for Trials — six invented, original scenarios (45–75 words each), every one with the accepted Who/What/Where/When/Why/How answer written out underneath, ready to use or swap out
- WH-Question Caseload Roster — 14 rows: student, trial type(s) used, current setting, last probed, next probe due
- Binder Cover and Divider Tabs — a spine/cover card, 4 cut-out tabs matching the file's sections, and an 11-row "where to find things" index
Take The Data
- Listening Comprehension Trial (oral WH questions) — a reusable 12-row log: scenario/question used, WH type, the student's verbatim answer, and the mark; built to be refilled across many sessions rather than reprinted each time
- Independent Reading WH-Question Check — the one student-held page in this file: a "My name"/date header, a pencil-and-reference instruction, a blank area for that session's passage, and six numbered answer boxes with real room to write. Nothing adult-facing appears on it — no goal text, no scoring strip, no marks
- Quick Spoken WH-Question Check — 12 fixed items, two per WH type, each a one-sentence prompt with the question to ask and the expected answer printed for the adult, for a fast check between full sessions
- Frequency Correct/Incorrect Chart — one row per trial type (3 rows) tallying presented/correct/incorrect/self-corrected/no-response for the session, plus an optional 4-session quick-trend table
- Paraprofessional and General Education WH-Question Check — two cut-apart copies, plain "Got it / Needed a repeat / Not yet" boxes, no codes, for the adult in the room who is not the case manager
- Answer Key and Scoring (adult only) — a 6-row scoring strip, one row per WH type, with the rule for determining the accepted answer from whichever passage was actually used, and a Total Correct / 6 box; never handed to the student
Track, Report And Share
- WH-Question Accuracy Tracker — two 12-session blocks (oral listening trial, and an optional independent written-check block), with date, percent correct and setting logged each session
- Progress Graph — a scaled 0–100 grid to plot the trend, with an aim line
- Quarterly Progress Summary — 6 rows, one per WH type, with baseline, current level and one report sentence, plus settings, percent then/now, and two write-on cards
- Family Update — Answering WH Questions — a ready-to-send communication page: what was practiced and which WH type, something that went well, one thing to try at home, family questions and answers, and a 7-day practiced table
- Progress Note Phrase Bank — 19 observable-language starters across 4 groups (accuracy by WH type, trial-type comparison, next steps, the quarterly-report sentence), every one ending in a blank pulled from a data sheet in the file
Plus a Start Here quick-start, a 3-section contents page with real page thumbnails, 3 section dividers, and a "what's next" back page.
What This Is, And What It Is Not
It is a fillable PDF. The goal, codes, bank, roster, log, tracker, graph, quarterly and family pages are type-in; the independent reading check prints cleanly and holds nothing but the passage and the questions. It works offline with no account or subscription. It is NOT a Google Sheets file, a Google Doc, or an Excel workbook, and it does not score a trial against a published norm or compute a percent for you.
It is not WH Question Reading Comprehension Progress Monitoring Probes — that file is six fixed, equivalent, scored forms meant to be rotated weekly for progress-monitoring accountability. This file is the ongoing trial-log companion: an oral listening trial, an independent reading check, and a quick spoken check, all built to be reused across many sessions rather than consumed form by form, with no passage, scenario or form shared between the two files. It is also not either of this store's picture-based WH-question products — every scenario and passage here is text, for a reader or listener, never an image — and it is not a visual cueing aid; nothing here prompts the student toward an answer. No page in this file names a condition, states an age or a milestone, or sets a rule for when a referral is warranted; every criterion is a blank line the team writes in from the IEP and its own judgment. Every scenario is an informal, invented example built for this file, not a standardized instrument. No health data is pre-filled anywhere, and the example student (Amara) is invented and named as such.
Who This Is For
Speech-language pathologists, special education case managers, and the paraprofessionals and general education teachers who help collect WH-question comprehension data.
If You Want More
Auditory Processing and Discrimination Data Sheets Editable is the other trial-log binder in this cluster — sound and sentence discrimination, memory and directions, not WH-question comprehension. WH Question Reading Comprehension Progress Monitoring Probes is the six-form scored probe, for teams who want a fixed weekly battery instead.
