About this resource
ABC data, and nothing else. Five layouts of the same sheet, because the sheet that works for a scheduled observation is not the sheet that works while you are teaching: a narrative ABC sheet, a checklist sheet where you tick the codes, a quick sheet you can finish in ten seconds a row, a multi-student sheet, and a summary sheet that turns a week of rows into counts. Type the behavior definition and the code key once, Save As per student, and pen the rows.

Setup
- How to Take ABC Data - a 6-row chooser that matches the room you are actually standing in to one of the five layouts, the difference between observable words and a judgment, what the codes are for, and the weekly habit that makes the rows worth taking

- Code Key and Behavior Definition - three write-in blocks for the observable definition, the examples and the non-examples, then seven antecedent codes and seven consequence codes, each with a type-in column for what that code means in your room
Five Abc Layouts
- ABC Data Sheet (narrative) - 8 tall rows: date and time, setting, antecedent, behavior in observable words, consequence, length or intensity, codes
- ABC Checklist Data Sheet - 14 rows with the seven antecedent codes and the seven consequence codes as tick columns, three words for the behavior, and a length column
- ABC Data Sheet, Quick - 16 short rows: time, setting code, antecedent code, behavior in three words, consequence code, initials, with a block to write your own setting codes
- ABC Data Sheet, Multi-Student - 12 rows with a student column, for one room, one block or one route where several students are being recorded at once
- ABC Summary Sheet - one behavior, one week: a tally and count cell for each of the seven antecedent codes and each of the seven consequence codes, a 7-column tally of which hour of the day the rows fell in, and three fields for the highest antecedent code, the highest consequence code, and the pair that repeats
Example
- A filled-in week for an invented first grader: eight quick ABC rows, the code counts and hour counts added up from exactly those rows, and a short read of what the counts do and do not say
What This Is, And What It Is Not
It is a fillable PDF. Open it in a PDF reader, click a line, a box or a cell, type, press Tab to move on, and Save As once per student. It works offline with no account or subscription. It is NOT a Google Sheets file, a Google Doc or an Excel workbook, and it does not add up the code columns for you.
It is not a behavior plan and not a functional behavior assessment. The antecedent and consequence codes are for recording what happened before and after a row so that a week can be counted; a tall column is a place to look next week, not an answer. Anything your team has concluded belongs on your team's own paperwork. Nothing here describes a procedure, a label or a timeline.
