About this resource

An incident log records the facts: when, where, how long, who was notified, who marked it. Not a clinical tool and not an intervention guide — just the observable record that a team needs to see the pattern. One row per incident, one sheet per week, column headers built in so the team always records the same information. No ABA methods, no de-escalation procedures, no diagnosis language. The district's own behavior plan gets a blank line.

It is a fillable PDF. Open it in a PDF reader, click a cell, type the date or time block or setting, press Tab to move on, and Save As per student or per week. It works offline with no account or subscription. It is NOT a Google Sheets file, a Google Doc, or an Excel workbook, and it does not total, count, or calculate a threshold for you — you add the counts by hand, and the summary pages show the addition so it can be checked.

It is not a functional behavior assessment. It is not an ABA data sheet, and it does not describe intervention or de-escalation procedures — every approach gets a blank line for the team's own district rule. This file never analyzes why an incident happened or what caused it. It is a record of observable facts: when, where, how long, who was told. The team's own analysis and the district's own plan are written elsewhere.

Special education case managers, general education teachers, and school support staff who want to track when aggressive-behavior incidents happen and where, so a pattern shows without reading each day's narrative. A classroom teacher fills the log as incidents happen; an administrator rolls them up weekly for the team; a family member can help track incidents at home by filling the "home" column.
