About this resource

Track bathroom visits as the day happens — the time, how long, the independence level shown, and the setting (school, home, community). This file rolls daily data into weekly and monthly summaries and turns it into a one-page family update every month. Data only: frequency, duration, independence level. No toileting protocols or training procedures; every district rule gets a blank line to write it on.

It is a fillable PDF. Open it in a PDF reader, click a line, a box or a cell, type, press Tab to move on, and Save As per student or per month. It works offline with no account or subscription. It is NOT a Google Sheets file, a Google Doc, or an Excel workbook, and it does not train independence or set the protocol for you.

It is not a toileting training procedure (that is the team's own plan, written on page 5). It is not clinical guidance on physical or medical procedures — every approach gets a blank line for the team's own district rule. It is not a behavior tracker (see our Behavior Data Collection Sheets for frequency, interval, and ABC counting). It is a plain bathroom visit: when, how many, how long, and the independence level shown. Every decision point on every page ends in a blank for the team's own criteria.

Special education case managers, general education teachers, and special-education teachers logging bathroom independence across a school day and across time — and any paraprofessional, aide, or related-service provider who is asked to log bathroom visits. School and home tracking can run side by side; a family can use it to track home routines and share data with the school.
