About this resource

This is a grid, not a log. Every working page is rows by columns with one tracking code per cell — the whole class for a week, one student for a month, morning versus afternoon, school versus home — so a case manager or related-service provider sees the pattern at a glance instead of reading a day of time-stamped rows. Data only: a code per cell, never a procedure or a clinical conclusion. The district's own protocol gets a blank line.

It is a fillable PDF. Open it in a PDF reader, click a cell, type one letter, press Tab to move on, and Save As per student or per month. It works offline with no account or subscription. It is NOT a Google Sheets file, a Google Doc, or an Excel workbook, and it does not total, average, or calculate a percent for you — you add the counts by hand, and the summary pages show the addition so it can be checked.

It is not the bathroom log (that is a separate product: a chronological, one-trip-at-a-time record, for when the exact time and sequence of a single day matters). This file never records a single visit by time; every page is a grid of codes read at a glance. It is not a toileting training procedure and not clinical guidance on physical or medical procedures — every approach gets a blank line for the team's own district rule.

Special education case managers, general education teachers, and related-service providers who want a week's or a month's bathroom pattern at a glance instead of a day of narrative rows — and any paraprofessional or aide marking a grid at school. School and home grids run side by side, and a family can help track the home column.
