About this resource
Eight behavior contracts written with the student, not for them, in one fillable PDF: a simple primary contract, a standard elementary contract, a secondary contract, a home-school contract, a self-management contract, a group and class contract, a work completion contract and an attendance contract, plus the teacher's planning sheet, a ten-day check-in sheet, a weekly review that keeps, changes or ends the contract, a reward menu with a reinforcer survey, a caseload tracker, a family letter, and a filled-in example. Type the contract with the student in the room, print it, sign it in pen.
WHAT IS INCLUDED - 20 pages, 729 type-in fields
Setup
- Which Contract for Which Student - a nine-row chooser that matches the student and the situation to a layout, the four parts every contract has (the behavior in observable words, what the student earns, how it is checked, when it is reviewed), how a contract is written with the student, and how the paper moves before the meeting, on signing day, and every day after
- How the Type-In Fields Work - one page, so nobody emails you
- Contract Planning Sheet - the teacher's page, twelve numbered parts: target behavior in observable words, replacement, where and when, how it is measured, the reward menu, who signs, review date, layout, fade plan, the student's own words, the goal number, who checks
Eight Contracts
- Simple Contract - for primary students: I will, my teacher will, when I do this I earn, we will check on, a box for a picture, big lines, two signatures
- Standard Behavior Contract - the elementary layout: my goal, what it looks like, what I earn and its cost, a three-row check-in schedule, points needed, review date, student / teacher / family signatures
- Secondary Behavior Contract - up to three goals with a criterion each, four privilege rows with a cost in points, how I want to be reminded, two review dates, four signature lines
- Home-School Contract - five parts with a school column and a home column (the behavior, what the adults do, what the student earns, how we communicate, who checks), three week-by-week rows, three signatures
- Self-Management Contract - the student writes the goal, the plan and the check, rates each of five days 1 to 4 beside the teacher's rating, and totals the week
- Group and Class Contract - the class agreement in the students' words, eight expectations with looks like and sounds like, sixteen signature lines
- Work Completion Contract - twelve assignment rows (due, done, turned in, teacher check) with a weekly total counted against a goal number the student agreed to
- Attendance and Punctuality Contract - two weeks of Monday to Friday: arrived on time, stayed the day, a note, initials; goal in days out of ten
Check-In, Review, Rewards, Tracking, Home
- Daily Check-In Sheet - ten days: met or not, points earned, points possible, initials, note, and a total row that the contract's goal is checked against
- Weekly Contract Review - what worked, what did not, the week's numbers, and one of three decisions (keep, change, end) with what changes written down; the next review date and two signatures
- Reward Menu and Reinforcer Survey - twelve choices the teacher types, the student's rank, a cost in points, when it can be used, a four-pair survey, and a list of no-cost ideas
- Contract Tracker - twelve rows for the caseload: student, layout, started, review date, status, menu on file, family told
- Family Letter About the Contract - plain words with typed blanks, no warnings, and a reply slip that comes back with one line from home
- A filled-in example for an invented fourth grader: a standard contract and ten check-in days whose total re-adds against the goal, with the two Friday reviews
What This Is, And What It Is Not
It is a fillable PDF. Open it in a PDF reader, click a line, a box or a cell, type, press Tab to move on, and Save As once per student. It works offline with no account or subscription. It is NOT a Google Sheets file, a Google Doc or an Excel workbook, and it does not add the points or check the goal.
It is not a behavior plan, a discipline policy or a list of strategies. It does not say which students should have a contract, what the goal number should be, or what happens when a contract is not met; each of those is your team's decision, and each has a blank to write it on. It is the paper a contract is written on, so that the goal is in observable words, the student picked the earn, and the check-in happens the same way every day.
Who This Is For
Special education teachers and case managers, behavior interventionists, school counselors and social workers, and general education teachers who have been asked to write a behavior contract with one student, or with a class.
