About this resource
Most behavior plans are a list of consequences with a cover sheet. No definition a stranger could count, no hypothesized function, no replacement behavior that gets the student the same thing, no way to tell whether the plan was ever actually done.
This kit is the worksheets that force those parts to exist, in the order the thinking has to happen, as a fillable PDF: define, assess, plan, measure, review, and then the one-page summary that the adults in the room will actually read. Type it once, Save As per student.

Define And Assess
- Target Behavior Definition - observable words, examples, non-examples, and a baseline measured as frequency, duration, latency or intensity.
- FBA Summary and Hypothesis - data sources, setting events, antecedents, consequences, the hypothesized function (attention, escape, access, sensory) and a hypothesis statement frame.
- Function-to-Strategy Map - what each function means for prevention, replacement and response, with a column for this student.
Plan
- Replacement Behavior Plan - with the five-point check that it serves the same function and is easier than the target behavior, plus teaching, reinforcement and fade.

It is not a legal template and it does not say what your state or district requires a behavior plan to contain. It is not a substitute for your district's FBA process or for a behavior analyst where one is required. Nothing in it describes or authorizes any physical intervention; safety procedures are your district's, and the safety page only records where they live. If your district has its own BIP form, these pages are the working paper behind it.
