About this resource
Six ways to walk into a room and come out with something the team can use, before a referral goes anywhere or a plan gets reviewed: a narrative form, an interval sheet with a comparison peer built in, a plain interval recording sheet, a scatterplot, a classroom setting review, and a teacher interview form. Every page carries the same rule: observable words and a time stamp, never a conclusion. Type the student and the setting once, Save As per visit, and pen the observation itself.
WHAT IS INCLUDED - 12 pages, 619 type-in fields
Six Observation Layouts
- Narrative Observation Form - a header of student, observer, date, setting, activity, start and stop, then 14 time-stamped rows: what the student did, what happened right before, what happened right after, what others did
- Interval Observation with Peer Comparison - 20 intervals across a target student and an unselected comparison peer, on-task yes or no for both, a same-or-not row, and a totals table with intervals on-task, intervals observed and percent on-task for each
- Interval Recording Sheet - 13 observations by 10 intervals, partial, whole or momentary (write which), a plus count and a percent
- Scatterplot - 15 half-hour time blocks by two weeks of school days, shaded by how much happened
- Classroom Setting Review - 16 things about the room itself, grouped under seating, materials, transitions, adult proximity, pacing and noise, each with a type-in description and a present-or-not column
- Teacher Interview Form - 12 questions with answer lines, for the classroom teacher's own account when the observer was not in the room every day
Summary
- Observation Summary - a log of every observation this cycle (date, form used, minutes, what was seen in counts and percents, observer), fields for the totals across the cycle, and two write-in cards for patterns and questions for the team. The first row is an invented example so the format is clear; every number in it is computed and checked in the module's own code, and the page says in plain words that the row is invented.
What This Is, And What It Is Not
It is a fillable PDF. Open it in a PDF reader, click a line, a box or a cell, type, press Tab to move on, and Save As once per visit. It works offline with no account or subscription. It is NOT a Google Sheets file, a Google Doc or an Excel workbook, and it does not add up interval marks or shade the scatterplot for you.
It is not a functional behavior assessment, not a behavior intervention plan, and not a tool for assigning any label. No page states or implies why a behavior happens; every form asks only for what happened and when. What a set of observations adds up to is for the team to decide together, on the team's own paperwork.
