About this resource

Career interest survey you can type on and print, for grades 6 to 10. A student marks what they like, counts it, and writes their own next step. This is 18 pages: 9 student pages and 6 teacher pages, plus a cover, terms of use and a thank-you page with credits.

How To Edit

Open the PDF in Adobe Acrobat Reader, Preview or your browser. Click a box or a line and type, then Save As. 242 type-in fields in all. This is a fillable PDF, not a slide deck, so there is nothing to ungroup or unlock. Print it at 100 percent, or hand out the student pages and let the student mark them by hand.

What This Is, And What It Is Not

A survey the student marks, a summary the student counts, and teacher pages to roll it up. It is not a test. It does not score a student, and it does not choose a job or a program. The student and the team decide what the answers mean. The example student, Jordan, is invented and labeled that way on the page.

It is the student's own survey for grades 6 to 10. It is not the teacher's data sheet: Secondary Vocational Interest and Skills Assessment Data Sheets Editable is a separate file for grades 9 to 12, where the teacher records observations over time.

Who This Is For

Special education teachers, transition coordinators, school counselors and case managers who start a career conversation with students in grades 6 to 10, and who want a student to mark their own interests.
