The whole Check-In Check-Out routine on paper, in one fillable PDF: a setup sheet and roster, four point-card layouts, weekly and monthly data sheets, a progress graph, the coordinator's daily log, a weekly team review with the four decisions written in plain words, a fade and graduation plan, a parent letter with a weekly home note, a student self-rating card for the fade phase, and a filled-in example whose every number re-adds. What This Is, And What It Is Not
It is a fillable PDF. Open it in a PDF reader, click a line, a box or a cell, type, press Tab to move on, and Save As once per student. It works offline with no account or subscription. It is NOT a Google Sheets file, a Google Doc or an Excel workbook, and it does not add the points or draw the graph.
It is not a program manual and not a summary of anyone's findings. It does not say which students should be on Check-In Check-Out, what the daily goal should be, or how many weeks at goal come before a fade; each of those is your team's decision and each has a blank to write it on. It is the paper the routine runs on, so the same card is rated the same way in every room and the Friday number can be trusted.
