About this resource

MTSS coordinators, general education teachers and special educators are often asked to identify students who may need Tier 2 support without any data to start from — just a hunch about who to watch. This file is a classroom-wide universal screening system: rate every student in the room, once per cycle, on three observable domains, and turn the ratings into a short, defensible list for the team to look at next.

This is a screening TOOL, not a screening RESULT. Every student is rated 1–5 on three domains — Engagement, Behavior, and Emotional Regulation — using observable anchors (what a rater saw, not what they know about a student generally). The three scores add to a total from 3–15, which sorts into a Low, Watch, or Elevated band. **Every band is described as a signal for your team to review, never an automatic trigger, a placement, or a finding.** The scoring guide spells out exactly what separates a 3 from a 4 on each domain, in observable terms only.
