About this resource

A daily behavior chart you build yourself: type your own periods, your own goals and your own point values once, then print it and mark it every period. Three chart layouts cover the goals that come up most — periods by goals with a points row for a goal checked several times a day, a half-day chart for a simpler morning/afternoon goal, and a single-goal frequency tally for a behavior that is counted, not scored — plus a setup page, a points-to-reward menu, a home copy with a caregiver signature line, a weekly summary, and a filled example for an invented student.

Every chart in this file uses the same periods, goals and point values typed once on the setup page, so moving a student from one layout to another does not mean starting over.

What This Is, And What It Is Not

It is a fillable PDF. Open it in a PDF reader, click a line, a box or a cell, type, press Tab to move on, and Save As once per student. It works offline with no account or subscription. It is NOT a Google Sheets file, a Google Doc or an Excel workbook, and it does not add the points or choose the reward.

It is not a behavior plan and not a point system chosen for you. Every point value, every goal and every reward on the menu is typed in by the adult who knows the student; the chart only makes sure the same mark happens the same way, in the same place, every period, and that the numbers reach the weekly summary. It has no icons or picture library — every page is text, fields, and tables.
