About this resource

Exit Ticket Template: a fast, end-of-lesson check for understanding, for any classroom teacher, any

grade, any subject. Every question is yours to write in — this file supplies the format, never the

content of today's lesson.

WHAT IS INCLUDED — 16 pages, four exit ticket formats, a substitute-friendly version, a whole-class

roll-up, a team hand-off note, a 30-starter phrase bank, a weekly prompt planner, and a filled example

(you write in the question and the four answer choices), bubble answers A through D.

What This Is, And What It Is Not

It is a fillable PDF: open it in a PDF reader, type into the lines and bubbles, and save. It also

works printed and photocopied in black and white — nothing on the cut-apart slips depends on color

to be read. It is not a Google Sheets file, a Google Doc, or a slideshow file, and it does not grade

or score an answer for you.

It is not a homework tracker, a behavior chart, or a skill probe with a fixed item bank. There is no

answer key and no permanent record built in — every question line is blank, because an exit ticket's

content changes with whatever was just taught. It is a same-day snapshot of who understood today's

lesson, kept by any classroom teacher, specials teacher, substitute, or paraprofessional.

Who This Is For

Classroom teachers at any grade level, specials and related-arts teachers, substitutes, co-teachers,

interventionists, and paraprofessionals who want a ready-made way to check understanding at the end

of a lesson without building the form themselves.
