About this resource
A student has a moment that needs fixing. Then what? This ticket gives them one short, clear way to name the part they want to fix, pick a fix, and plan for next time. It is on paper, so a child can finish it at the desk in a few minutes, and an adult can read it at a glance.
EVERYTHING INSIDE (22 pages)
For The Student
- 1 picture ticket: three parts, six choices in each part, each choice with a picture and a check box
- 1 words ticket: the same 18 choices in words, with a My own line for each part
- 3 cut-apart sheets: six pieces a sheet, 18 pieces in all, cut once and glued on the glue ticket
- 1 glue ticket: three big boxes, one for each part
- 1 write-it page: a starter bank and three sentence frames with ruled lines
- 1 plan page: First, Then and Last boxes with room to draw
- 1 desk strip: four steps (Stop, Check the part, Fix it, Next time), each with a picture
- 1 tracker: 20 circles (4 weeks of 5 days) and a line for what the student is proud of
FOR THE ADULT (including the cover, the terms and the credits)
- Start Here, a how-to-use page with the six codes, and a talk-it-through page with three questions in order
- A Fix It Log: 16 rows, the codes for each part, and next time done (Yes, Partly or No)
- A weekly roll-up for four weeks and the month, with a percent-yes column
- A note home for families
- A filled example for an invented student, Jordan: one ticket, a log of ten tickets over two weeks, and a roll-up whose every number is checked against that log
What This Is, And What It Is Not
It is a print-and-cut set. It is NOT editable: there are no type-in fields, so print it and write by hand. It is NOT a school policy, a behavior plan, a consequence system or a clinical tool. What happens after the ticket is your team's decision, and the page for it has a blank line. Every choice belongs to the student, and there is no answer key.
Who This Is For
Teachers, aides and counselors in grades 1 to 5 who want the same short routine every time a student needs to fix something. The student pages work on their own at a desk; the adult pages keep the record in the same six codes.
