About this resource

How many times, or how long — the two most common ways a behavior goal is written, as a fillable PDF. A frequency count sheet and a 20-day frequency tally for one behavior, a duration/latency sheet and a multi-episode timing log, an interval recording sheet for behaviors too frequent or continuous to count one at a time, a graph, and a filled example. Type the student and headers once, Save As per student, pen the tallies and times.

What This Is, And What It Is Not

It is a fillable PDF. Open it in a PDF reader, click a line, a box or a cell, type, press Tab to move on, and Save As once per student. It works offline with no account or subscription. It is NOT a Google Sheets file, a Google Doc or an Excel workbook, and it does not add up the tally columns or draw the graph for you.

It is not a behavior plan or a functional behavior assessment, and it does not decide which goal wording to use. It records a count, a rate, a duration, a latency, or an interval outcome, one row or one mark at a time. Whether a number counts as progress toward a particular goal is your team's own decision, made with your team's own paperwork.

Who This Is For

Special education teachers and case managers whose goals are written as a count or a duration, the paraprofessionals and general education teachers who take the data day to day, and behavior specialists who want the same two sheet shapes used the same way across every classroom on a caseload.
