Special educators serving students with moderate to significant support needs are asked to show progress across functional and adaptive-behavior domains — self-care, household tasks, community skills, money and resource management, safety and health decision-making, leisure and recreation — as an ongoing measure, not a one-time inventory. This file is that ongoing measure: rate each domain monthly on an invented four-level independence scale, log individual sessions in detail, check whether a skill has generalized outside the room it was taught in, and roll the month up into a report and a family update that write themselves.

Set Up The Caseload

What This Is, And What It Is Not
It is a fillable PDF. Open it in a PDF reader, click a field, type, Save As once per student. It works offline with no account or subscription. It is NOT a Google Sheets file, a Google Doc, or an Excel workbook, and it does not calculate a percent or draw the graph for you.
It is not Life Skills and Daily Living Data Sheets Editable — that file is a curriculum and planning pack built to TEACH a routine step by step (task-analysis layouts, a routine checklist, domestic and vocational job sheets). This file is the ongoing progress-monitoring companion: how independent a student is getting across six functional/adaptive-behavior domains, rated monthly and rolled up into a report — no task-analysis layout, no routine-teaching content. It is also not IEP Goal Tracking Editable, the general classroom trial-data file for any goal; use this file when the goal is written in terms of independence in a daily-living, community, safety or leisure domain specifically.
The four-level independence/support scale is invented for this file, not a standardized or clinical instrument. No page states what a student should be able to do at any age, names a condition, or sets an eligibility or IDEA-timeline rule; every criterion is a blank line the team writes in from the IEP and its own judgment. No health data is pre-filled anywhere, and the example student (Jayden) is invented and named as such on page 22.
