About this resource
EVERYTHING INSIDE — 22 pages
- Two sets of student goal pages. Grades 3-5 (pages 5-8): My Goal, My Steps Up, My Check-Ins and Looking Back, big and drawn, with primary writing lines and room to draw. Grades 6-12 (pages 9-12): My Goal, My Action Plan, a Weekly Check-In Log and Looking Back, calmer, with room for sentences. Every student page has a My name line and one short instruction
- 24 goal cards to pick from and cut: 12 for grades 3-5 on page 13 and 12 for grades 6-12 on page 14. Each has a picture, one plain sentence and a line to finish "My goal". They meet edge to edge, so a page is two cuts down and three across
- My Steps Up: four numbered steps to climb to the goal, with a box for each and a box for who can help
- My Check-Ins: six weekly rows with a date, how many times, what helped, and five stars to color
- An action plan table: 9 rows for the step, what I will do, by when and done
- A weekly check-in log: 12 rows for grades 6-12
- I Did It!, a celebrate page a child fills in and draws on
- A class goal tracker with a roll-up: 13 student rows, steps set, steps done, check-ins and reached, plus class totals
- Goal Talk: five questions for a short one-to-one talk, with room for the student's words
- A note home with blanks in your own words
- A phrase bank of 32 starters in 8 groups
- A filled example: one invented student's goal, six weekly check-ins and a five-student class tracker, every total re-added from its own row
- A binder cover, a spine label and 8 cut-apart tabs
- Start Here, a contents page with page thumbnails, and a "what's next" back page
A goal setting template students can actually use. Most goal sheets are one page for an adult to fill in. This one gives the student the pen: pick a goal, climb the steps, check in each week, look back, and celebrate, with a tracker and a talk page for you.
What This Is, And What It Is Not
It is a fillable PDF: type into the pages, or print them and write by hand. It works offline with no account or subscription. It is NOT a Google Slides, Google Docs or PowerPoint file, and you cannot drag the boxes around or restyle the pages.
It is working paper for everyday classroom goals a student chooses. It is not an IEP goal page and does not restate, replace or score any goal your team has written. The tracker counts steps and check-ins; it does not grade a student. No health information is filled in. The example students are invented and say so on the page.
