About this resource
Which assignments were due this week, did the student turn them in, and are they being completed. This is a plain
data tracking sheet for homework status, not a gradebook and not a proficiency rubric: there are no letter grades,
no scoring strips, and no automatic calculation. Every completion code and due-date rule is blank for your
district's own practices and policies.
WHAT IS INCLUDED — 16 pages, five tracking sheet formats, a caseload roster, setup guides, a home-school communication
note, a progress snapshot, a 30-starter phrase bank, and a filled example
Five Tracking Sheet Formats
- Individual Weekly Homework Tracker — 18 rows, one per assignment: subject, assignment name, date assigned,
date due, turned in (yes/no), completion code, and notes. The detailed version for close monitoring.
- Quick Homework Check-In Log — 15 rows (covers three five-day weeks on one page): subject, assignment, turned in
(yes/no), notes. No completion-code column, for when there is no time.
- Monthly Homework Overview — 18 rows, one per week (roughly a semester on one page): assignments assigned, assignments
turned in, completion rate (%), patterns or notes. The big-picture view for monthly check-ins.
- Home Homework Communication Note — a one-page letter home with a five-subject status table, what assignments are
outstanding, what was completed, completion rate for the week, how families can help at home (five prompts), a
place for questions, and a Response From Home section (note, preferred contact method and time, interpreter-needed
checkbox, and a signature/date line).
- Resource Room Homework Check — a resource-room or study-skills teacher's version. Track what homework arrives from the
general-ed classroom and what the student completes during the resource period. 16 rows.
Reporting And Communication
- Homework Completion Progress Snapshot — for a team meeting or progress note: an 8-week completion-rate table, target
vs. actual rate and the gap between them, overall trend (growing, steady, declining, needs support), what changed
since the last check, strengths, next steps, an attachments-reviewed checklist, and a prepared-by/date line.
- Homework Observation Phrase Bank — 30 observable-language sentence starters across six categories (homework arrival and
setup, engagement and effort, completion and accuracy, support conditions, progress and patterns, documentation and
communication), every one ending in a blank for what was actually seen.
- A Homework Tracker, Filled In — an invented fourth grader, one week of homework across six subjects (eight
assignments total), with a completion-code breakdown table and a summary whose every number is checked against
the table rows.
- Binder Cover and Divider Tabs — a cover page and four divider tabs (Setup, Tracking, Summary, Home & Reference) with
a "what goes behind each tab" guide and a binder setup checklist, to print, cut, and drop into a three-ring binder.
What This Is, And What It Is Not
It is a fillable PDF: open it in a PDF reader, click a line or a cell, type, and Save As once per student per week.
It works offline with no account or subscription. It is not a Google Sheets file, a Google Doc, or an Excel workbook,
and it does not count or calculate for you.
It is not a gradebook (no letter grades, no scoring rubrics, no class rosters). It is not a proficiency scale (no
"proficient," "passing," or "mastered" language). It is one student's homework completion tracker, kept by a case manager,
resource teacher, parent liaison, or home-school coordinator. The completion codes, due dates, and target rates are blank
lines for your district's own practices, not invented by this file. You supply the completion status based on what your
team has decided; every school's completion code is different.
Who This Is For
Special education teachers, case managers, resource teachers, study-skills instructors, parent liaisons, families,
and paraprofessionals supporting a student's homework completion and assignment turn-in.
