About this resource

An IEP accommodations tracking file for the part that happens after the plan is written: what was actually provided, each school day. Copy the accommodations from the student's plan onto one setup page, then mark each one every day as provided, not needed that day, or refused by the student. Count the marks at month end, note what helped, and bring the numbers to the team. Everything is blank for you and your team: this file lists no accommodation and never says which ones a student should have.

WHAT IS INCLUDED - 18 pages: setup, a weekly log, a month grid, a class-period check-in, a sheet for another teacher, a monthly summary, a what-helped page, a caseload overview, a team or family update, a student page, a phrase bank, a filled example, and a binder cover with tabs

What This Is, And What It Is Not

It is a fillable PDF: open it in a PDF reader, click a box, type a letter, and Save As once per student. It also prints cleanly in black and white. It works offline with no account or subscription. It is not a Google Sheets file, a Google Doc, or an Excel workbook, and it does not add up marks for you.

It is NOT a list of accommodations and gives no advice on which a student should have: the IEP or 504 team decides that. It prints no rule or timeline, and it does not score or judge anything. Example students are invented.

Who This Is For

Special education case managers, resource and self-contained teachers, 504 coordinators, and the general education teachers and paraprofessionals who provide accommodations day to day.
