About this resource
You want the whole month on one page, one box for every school day, and a goal the student can say out loud. This is a fillable PDF with a chart for each month of the school year, plus the pages the student and the family see.
EVERYTHING INSIDE — 25 pages
- Start Here, a contents page with page thumbnails and a "which page for which job" chooser
- Our Goal This Month: room for 3 goals in your words (the goal, what it looks like, what it sounds like), the three marks, a reward line and a blank for your school's own rule
- 12 month charts, August to July, one page each. Every chart has a grid of 6 weeks, Monday to Friday (30 day boxes), a date box and a mark box for each day, a goal line, and Y days, S days, N days and School days totals. The dates are typed in, so the same file works in any year
- My Month: a page the student fills in, with a big My name line, a month line and 25 stars to color
- My Month Check: a page the student fills in, with a face to circle for each of 5 weeks and two writing boxes
- A class roll-up for the month: 17 student rows, 5 weekly columns and a month total
- A note home with blanks in your own words and a month-at-a-glance table
- A phrase bank of 32 starters in 8 groups
- A filled example: one invented student, one October, and a five-student invented class, every number re-added from its own row
- A binder cover, a spine label and 8 divider tabs (Our Goal, Aug - Oct, Nov - Jan, Feb - Apr, May - Jul, Student Pages, Class Roll-Up, Family Notes), edge to edge so the tabs cut apart in two straight cuts across and three down
The marks are plain: Y for yes, S for some of the time, N for not yet. The same three faces are on the student page, so the teacher's mark and the student's check line up.
What This Is, And What It Is Not
It is a fillable PDF: type into the charts, roll-up and note, or print and use a pen. It works offline with no account or subscription. It is NOT a Google Slides, Google Docs or Google Sheets file, and you cannot drag the boxes around or restyle the pages. It does not add up marks for you; there is a place on each page to write the totals.
It is working paper for counting what you see across a month. It is not a behavior plan, a grade, a point economy or a label for a child. The goal wording, the reward line and what your school expects are blanks for you to write. The student, the class and the teacher in the example are invented and say so on the page. No health information is filled in. Only the binder tabs (page 24) are cut.
