About this resource
Paras collect most of the data on a caseload, and most of the sheets they are handed were designed for the case manager. These were designed for the person holding the clipboard: goals in plain words, what counts written on the sheet, big cells sized for a pen held standing up, and a code key on every page.
The teacher sets each sheet up once on screen (student, goals, what counts), saves a copy, and prints a stack for the week. The para writes with a pen, or types straight in on a tablet. On Friday the sheets leave the clipboard on the hand-off page and reach the goal tracker.
WHAT IS INCLUDED - 16 pages, fillable
For The Teacher
- Before You Hand These to a Para - six things to do first, what paras can collect and what they should not be asked to, and how the paper moves.
- Para Schedule and Student Assignments - which student, when, where, which sheet, and who it goes to.
The Daily Sheets
- Daily Data Sheet - one student, one day, up to seven goals; the teacher fills the goal and what-counts columns once.
- Quick Trial Grid - up to seven goals by ten trials, + and -, with a plus column and a trials-given column so 7 of 9 is honest and 7 of 10 is not.
- Behavior Tally by Time Block - up to three behaviors defined in the box first, nine time blocks, and a what-was-going-on-right-before column.
- Prompt Level Sheet - twelve steps by six dates, I / G / V / M / P codes, and a steps-with-no-help row.
- Work Completion and Independence Log - started, finished, prompts to start, prompts to stay on, how much was done.
- Break and Regulation Log - asked for versus given, what came before, strategy, minutes, came back to work.
- Daily Home Note - positive first, plain words, teacher signs.
Weekly And Reference
- Friday Hand-Off to the Teacher - what was handed in, one thing that stood out, what the para needs, and a column for the teacher to date when it went on the tracker.
- What Counts as Data, and What Does Not - ten things that get written by accident ("had a bad day") beside what the teacher can actually use ("3 call-outs in morning meeting, 6 in independent math, 0 in specials").
- A Daily Sheet and Tally, Filled In - an invented second grader and an invented para, so you can see how short the notes are and how much they still say.
What This Is, And What It Is Not
It is a fillable PDF. Open it in a PDF reader, click a cell or a line, type, press Tab to move on, and Save As once per student. It works offline with no account or subscription. It does NOT calculate or total anything, and it is NOT a Google Sheets file, a Google Doc, or an Excel workbook. Most paras will print it and use a pen; that is what the cells were sized for.
It is not a training manual (that is my Paraprofessional Handbook) and not the case manager's tracking system (that is my IEP Goal Tracking Editable). It is the clipboard layer between the two. Nothing in it is a district form and nothing asserts a legal requirement.
Who This Is For
Special education teachers and case managers who supervise paraprofessionals, and the paras themselves. Pages 6 to 12 can be handed to a para after being shown twice.
