About this resource

Every family that arrives on conference day, and what they talked about with you: a caseload sign-in roster, a daily arrival log where families sign in with the time they came, notes on who they met with and what topics came up (coded A for academic, B for behavior, S for social, H for health), a per-student conference notes page, an attendance and topics summary, a topics tally that shows what matters came up most often across all conferences, a staff notes page, a family communication page, and a prompt bank for opening lines, questions, and closing statements. Type the roster once before conference day. Save As with the date.

What This Is, And What It Is Not

It is a fillable PDF. Open it in a PDF reader, click a box or line, type, Tab to the next, and Save As with the date. Type the roster once and the file runs on time and topic codes. It works offline with no account or subscription. It is NOT a Google Sheets file, Google Doc, or Excel workbook.

It is not a sign-up sheet where families claim a time slot before the conference (that is a different product). It is not a reminder notice sent to families before conference day (that is another product). This is the roster families sign when they arrive.

Who This Is For

Teachers running parent-teacher conferences, administrators coordinating a conference day, special education staff meeting with families about IEP goals and progress, grade-level teams who want to track what families talk about most, and anyone who needs to show who attended, when, and what was discussed.
