About this resource

A reward and sticker chart set for a child learning to use the toilet: three pages the child holds and colors (a 20-star chart, a week star chart, and a celebration menu of reward choices), a reward and goal planner for the team, and the charts the adults mark behind it. Write your team's own plan once, then every day is a box, a tally mark, and a star.

What This Is, And What It Is Not

It is a fillable PDF. Open it in a PDF reader, click a box or line, type, Tab to the next, and Save As once a week. It works offline with no account or subscription. It is NOT a Google Sheets file, a Google Doc, or an Excel workbook.

It is not a toilet-training method or a behavioral procedure, and it does not set a pace or a schedule for you. This is the chart that records a program your team already decided on. Your training approach, what counts as a success, what earns a star, your reward plan, and your goal all go on pages 5 and 6 (the blank lines); so does your plan if progress is slower than you expect. The phrase bank and celebration menu are starting points, not rules — what you actually say and reward is your choice.

Who This Is For

Special education teachers and instructional assistants running an active toilet-training program, case managers tracking progress toward a toileting goal, early childhood and self-contained classroom teachers, and families working on the same program at home alongside the school. Anyone who wants a chart the student can see progress on, not just a log an adult fills out.
