About this resource

A progress report template for any special education goal area. Write each goal's report in the same three steps: the numbers, what they show, and what comes next. Pages cover one goal in depth, one goal across four reporting periods, every goal for a student on one page, and a plain-words summary for the family. Everything is blank for you and your team: this file prints no goal, no target, no reporting rule and no deadline.

WHAT IS INCLUDED - 18 pages: setup, report pages, a family summary, teacher notes, a tracker, a student page, a phrase bank, a filled example, and a binder cover with tabs

What This Is, And What It Is Not

It is a fillable PDF: open it in a PDF reader, click a line or a cell, type, and Save As once per student. It also prints cleanly in black and white. It works offline with no account or subscription. It is not a Google Sheets file, a Google Doc, or an Excel workbook, and it does not total or graph anything for you.

It is NOT a goal bank and does not write goals. It prints no IEP goal, mastery criterion, reporting rule or timeline: your IEP team and your district write those in. It does not score, compute or label progress; every progress label is blank for your team. Example students are invented.

Who This Is For

Special education case managers, resource and self-contained teachers, related service providers, and the general education teachers and paraprofessionals who add to the report.
