About this resource
Which level a student is reading, how many words they read correctly per minute (WPM), and whether
they read with fluent expression and phrasing. This is a plain data tracking sheet for oral reading
fluency, not a timed benchmark probe pack: there is no scoring strip, no aim line, and no fixed
benchmark form in this file.
WHAT IS INCLUDED — 17 pages, five tracking sheet formats, a caseload roster, grade-level benchmarks,
a fluency mastery log, a progress snapshot, a family update, and a filled example
Setup
- How This Fluency Tracking System Works — three steps and what each sheet is for
- How the Type-In Fields Work — one page, so nobody emails you
- Reading Level and Series Guide — Guided Reading levels (A–Z+), Fountas & Pinnell, Lexile codes,
and grade-level expectations for end-of-year benchmarks in K–3
- Caseload Fluency Roster — 21 rows: student, level, WPM target, fluency status, last check,
next check due
Five Tracking Sheet Formats
- Individual Fluency Data Sheet — 20 rows, one per check: date, WPM (words correct per minute), accuracy %,
level (Guided Reading or F&P), fluency pass or needs support, and notes. The detailed version for
close monitoring.
- Quick Fluency Check-In Log — 22 rows, just the numbers: date, WPM, accuracy, level, and fluency pass.
No notes column, for when there is no time.
- Small-Group Fluency Tracking Sheet — one shared passage list, up to 4 students in columns, 16 rows,
WPM and fluency pass checked off as each student reads.
- Caseload Fluency Overview — 18 rows, one per student: current level, current WPM, target WPM, fluency status,
last check date. The whole caseload at one glance.
- Family Fluency Progress Update — a one-page letter home: current level, current WPM, trend since last month,
what to practice, a short FAQ, and a place for questions. Parents or guardians fill in their observations
and send back.
Reporting And Communication
- Fluency Mastery and Level Goals — a log, 14 rows: when each level goal is reached (three consecutive fluency
passes), with dates and notes on who the student reads with best.
- Fluency Progress Snapshot — for a team meeting, 12 rows: current WPM, trend over one review period, level
progression, fluency status, and a note on when to revisit the plan. No graph, counts only.
- Fluency Observation Phrase Bank — 35 observable-language sentence starters across six categories (reading
behaviors, fluency and expression, accuracy and problem-solving, support conditions, progress over time,
documentation and meeting notes), every one ending in a blank for what was actually seen.
- A Fluency Data Sheet, Filled In — an invented third grader, six fluency checks over five weeks, with a
progress snapshot summary and all numbers consistent with each other.
- Binder Cover and Divider Tabs — a cover page and four divider tabs (Setup, Tracking, Summary, Home) to print,
cut, and drop into a three-ring binder.
What This Is, And What It Is Not
It is a fillable PDF: open it in a PDF reader, click a line or a cell, type, and Save As once per student.
It works offline with no account or subscription. It is not a Google Sheets file, a Google Doc, or an Excel
workbook, and it does not count or calculate for you.
It is not the store's probe pack products (timed benchmark forms, goal pages, and graphs). This file has
none of those; it is the running record you keep between timed probes, or instead of them, for a student
whose reading program uses continuous monitoring rather than benchmark probes. It does not score or
graph automatically; you supply the WPM and accuracy from whatever fluency measure your school uses (one-minute
timed read, running record, benchmark probes, or a reading program’s built-in measure).
Who This Is For
Special education teachers, reading interventionists, SLPs (speech-language pathologists) working on fluency goals,
case managers, and the paraprofessionals and families who support a student’s reading progress.
