About this resource
Mark a student's oral reading, tally the errors and self-corrections, and work out an accuracy rate and a self-correction ratio — on paper, your own text, your own leveling system. This is a plain recording-and-scoring form, not a branded assessment system: no reading-level cutoff, no accuracy benchmark, and no age is set anywhere in this file.
WHAT IS INCLUDED - 18 pages, a how-to-run guide, five marking codes, four recording sheet formats, an accuracy and self-correction calculator, a caseload roster, a progress snapshot, a family update, and a phrase bank
Setup
- How This Recording System Works - four steps and a walk-through of which page to use for which situation
- How the Type-In Fields Work, Plus the Five Marking Codes - substitution, omission, insertion, self-correction, and told, each defined in plain observable terms with how to mark it; no publisher name anywhere
- How to Run a Running Record Guide - a blank reference page; write in your own text source and your own school's leveling system (this file sets neither)
- Caseload Roster - 21 rows: student, text source, accuracy rate, last reading date, next check due
Four Recording Sheet Formats
- Individual Running Record Recording Form - an 18-line numbered area to write or paste in your own text, a marking margin beside each line, and a tally row for total words read, errors, self-corrections, and told words. The full-detail, one-reading version.
- Quick Single-Read Version - 19 rows, no line-by-line text area: just date, text used, words read, errors, self-corrections, and a note. The fast version for when there is no time for full marking.
- Small-Group / Whole-Class Quick Running Record Log - 13 rows, one shared sheet, 4 students tracked side by side the same reading period.
- Caseload Accuracy Overview - 18 rows, one per student: accuracy rate, self-correction ratio, text used, last reading date. The whole caseload at a glance.
Scoring, Home, And Reporting
- Accuracy Rate & Self-Correction Ratio Calculator - both formulas shown blank (nothing pre-computed), fill-in boxes for each step, and an 8-row reading-by-reading history table.
- Home Reading Log - 10 rows for a family to log at-home reading time between checks (no marking or scoring on this page by design).
- Progress Snapshot - a 5-bar hand-shaded chart plus a 10-row table (reading date, text used, accuracy %, SC ratio, trend), built for a team or IEP meeting.
- Family and Team Update - a short note naming the text a student is reading right now, the accuracy rate and self-correction ratio since the last update, and what is planned next.
- Observation and Notes Phrase Bank - 19 observable-language sentence starters across five categories (text choice and engagement, reading behavior, conditions and support, patterns across several readings, progress across readings), every one ending in a blank for what was actually heard.
- A Running Record, Filled In - an invented second grader, an invented 63-word passage marked with all five codes, the tally row, and the accuracy/self-correction arithmetic worked out step by step.
- Binder Cover and Divider Tabs - a cover page and four divider tabs (Setup, Recording, Summary, Home) to print, cut, and drop into a three-ring binder.
What This Is, And What It Is Not
It is a fillable PDF: open it in a PDF reader, click a line or a cell, type, and Save As once per student or reading. It works offline with no account or subscription. It is not a Google Sheets file, a Google Doc, or an Excel workbook, and it does not calculate anything for you — you mark the reading, tally it, and work the two formulas out by hand.
It is NOT any specific branded leveling or assessment system, and it does not reproduce, name, or score against one. "Running record" is a generic term for this practice. Every text source, leveling-system name, and accuracy-rate criterion is left blank for you to fill in with your own school's own system.
Who This Is For
Special education teachers, general education teachers, reading interventionists, case managers, and the paraprofessionals and families who help mark or log a student's reading between checks.
